INTRODUCTION Meeting needs is an essential concept which comes under the Care Act and that purposely movesawayfromtheterm“providingservicesor resources”.Asitrecognisesthatthe requirements can be met in various forms and that what may meet the requirements of one individual may not meet the requirements of another individual in the same condition. In health and social care, the social worker plays a specialized role that is to aid the patients as well as their families to cope with their illness or diseases and disabilities. The main objective to have a social worker in health and social care is to prevent and decrease the various negative social as well as psychosocial conclusion of the illness. It also aims to empower and instruct ill people to utilize their resources. This report includes a case study ofperson named Ola Adeyemi, thirty- year-old man who is suffering from Down syndrome which is a genetic disorder. A disorder “Down syndrome” is caused by trisomy of 21 number chromosomes instead of having the usual two copies it includes the three copies of chromosome number 21. here in the given case study, includes the role of a social care worker to build the care plan for Ola Adeyemi. This particular care plan is based on the accomplishment of Ola's aim to become independent and accomplish development in his life(Akemoglu, Muharib and Meadan, 2020). MAIN BODY In the given case study of Ola Adeyemi, it has been identified that Ola Adeyemi is suffering from a disorder called Down Syndrome and living with his parents in Wembley. As Ola Adeyemi is dependent on his parents for some of his daily activities such as dressing. Ola never performs social activities such as cooking food, using public transport and job although he just attended school until the age of 18. Due to his condition, his parents are suffering from several issues such as mental wellness complications, physical conditions. It is because when a person is born with this particular disability, that person has to go with lifelong intellectual disability as well as the delayed in his developmental process. As it is a genetic chromosomal disorder and causing disabilities that including learning in a person. In some cases, this disorder also involves the enhanced risk of facing several mental health problems such as behavioural fluctuations, anxiety, depression and many more. It can also lead to heart-related problems with time in an individual such as memory loss, dementia and to cope with such problems. They need 1
support of other people to perform their daily activities and then become dependent on others for their particular activities(Akoglu and Dankl, 2021). To access Ola Adeyemi's care plan, a social care provider can make a care plan with the help of the APIE model framework so that the care provider can easily provide basic needs which will help to improve and make him independent. The APIE framework can help the social care provider to deliver high-quality care to the patient. This framework can take a practical approach and an attempt to tackle such difficulties which are generally connected with the patient. It is considered that this framework is generally an effective scheme to empower wellness to evaluate clinical performance. APIE MODEL While applying this, framework, there is a need to evaluate the needs which should depend on the qualitative and quantitative data. There should be an observation of data which should be quantitative like the physiological status of the patient there should also need to routine check-ups like getting body temperature, heart rate, blood pressure. There is also a need to identify the need of the patient which should depend on concern, expectation or data for the further fulfilment of needs. This model provides the information where it stands for assessing the problem, planning a response, implementing a plan and evaluating response. This model is specially used in nursing care practices. Through this model, there is a discussion about Ola's needs that are going to be discussed further (Bhandoria and et. al., 2021). ASSESSMENT: This is the element that helps to identify the problem which should be focused on the solution and can provide a solution to me. Through assessing medical history, it can help to evaluate Ola's healthcare needs that should be fulfilled for health improvement. In this step, there is a need to gather data that should be taken and evaluated through a different procedure like collecting information that includes weight, height, pulse, temperature and blood pressure. There is also a need to identify qualitative data which includes social background, mental state, family status and other factors. There are various ways to collect data like face-to-face interaction, interview techniques, measurements, observations, on basis of medical history and others. In this, there is a need to ensure from the patient side that evaluation or interpretation is correct. In this, it has been identified that there is a need to improve their communication skills which should be effective and can help Ola to communicate and share his view effectively. This should also be accountable and should be evidence-based to provide care and treatment. It has been evaluated that there is a need for person-centred care that can be effective for Ola and can fulfil his needs. In this, there is a need to take participation within an individual which is related with patient choice and their consent regarding intervention. There is a need for the person-centred care plan and social model of disability which can help to provide better outcomes. There is a need to plan for a goal that includes improvement in speech or motor skills. It is also need to enhance and improve Ola's awareness about self-care (How and et. al., 2019). Speech therapy- There is a need to plan for speech therapy which is effective and can allow getting better and effective understand that can influence an individual to improve their ability to think effectively instead of expressing. There is a need to provide care and support to better improvement in speech. This is effective in improving the language programme and can allow express himself in a better way. Social care worker need to take care that this therapy should work effectively and can allow getting better improvement in the language and speech. Care provider should also ensure that there should not be any type of harm to the individual during improving their speech(Jessel and et. al., 2020). 3
Providing education and learning-Itis the one that is more effective for Ola. This can help to make him learn about the taking own care. This can also allow Ola to work effectively and can get include in society. This is effective and helps individuals to make better relations with others in society. This can have more impact on individual and through providing education and learning, it can help Ola to focus on his diet and better health by own self. Many types of education that are highly effective and can provide better assistance to Ola to improve his knowledge and learning. This can also help to get better and effective skills to perform various tasks like getting travel own self in public transport and others(Kaczorowska and et. al., 2019). IMPLEMENTATION: This is next step in the whole process that is effective and crucial for health improvement after the planning step. There is a need to implement the plan with better effect. This can have a high impact on the process of planning. In this, social worker need to implement it effectively which is associated with the care plan. Social worker can implement a plan and make in practices that can allow them to take care of every step of implementing the plan with changes in the health of Ola. To implement such plans social worker needs to develop a therapeutic relationship withthepatientsothatthepatientcaneffectivelycommunicatewiththecareprovider concerning his requirements and also the choice of care. The therapeutic relationship is purely based on trust and respect. In this step the care provider should also provide patient-centred care as this approach can enable Ola to manage his health as well as the quality of life, thereby enhancing his physical and social health and gratification with care. This approach includes some components such as social worker can treat their patients with respect and dignity. In this, patients often lose their independence and the patient-centred care approach allows the patients to keep their dignity by honouring their wishes (As in the case of Ola). It also provides personalised aid and support as this care enables the patients to retain their freedom and also make patients fulfil their personal needs. In this implementation step, the social model disability approach is also important for Ola and the social worker should provide him as it helps him to identify the obstacles which make his life more difficult. By removing these obstacles,creates equality and also offers Ola to become more independent, control his life. A new strategy has been developed by the government for the learning disabilities of the patients. The social worker should implement such strategies which are about valuing people to enhance patient care. This strategy has its basic principle which is right, freedom, their choices and inclusion. can be used in the case of Ola Adeyemi by the social care provider. As this can help Ola to improve his well being and skills(Rigney and et. al., 2018). EVALUATION: Evaluation is the final step of the APIE framework and the care plan. The evaluation process is based on three types such as goal-based, process-based and at last result based. The main purpose of this step is to understand and find out the potency of the social care provider as it eliminates the care planning which is not required and is extra. During this step or phase, the social care provider will find out how to measure the success of the goals and interventions of the patient care. Their measures include the patient's condition which can be improved, stabilised and worsened. It is quite necessary to set a determinable goal so that a social worker can easily classify the patient's care plan. In addition to this, the social care worker also needs several skills which can help to improve the patient outcome such as Empathy, problem-solving skills, motivation, communication skills, self-awareness. Social care provider should understand patient need and feelings and the situations which the patient is facing(Opoku and et. al., 2019). Through communication skill social care provider can communicate with Ola effectively to fulfil his needs. It is quite important for care providers to understand the feeling of their patient. These effective skills can develop a strong relationship among patients and care providers that can lead to manage the medical conditions and improve their health behaviours. The skills such as Problem-solving can help in proceeding solution to patients problem which h involves the findings as well as how to deal with the problems that are arising with patient care. It consists the knowledge or understanding about a situation, identifying the cause, considering the situation and finally choosing the best possible outcome to cope with a particular condition. Motivation plays an important role in patient care as it is necessary to motivate their patients so that they can build their confidence and improve their well being. In the case of Ola Adeyemi, this typing skill is important as it can help him to make his relationship more comfortable, improve his behaviour and also make him independent. Self-awareness includes the awareness about own so that an individual can manage his/her emotions, align owns behaviour according to their values, make it understandable in front of others. People who are very self- aware about their own can easily interpret their emotions, their actions, thought processes and so on. Benefits or funding assistance: The benefits or funding assistance in patient care provides several resources to increase their well being. The social worker should provide enough information or knowledge to Ola about the free services provided by the government for disabled patients. It is necessary for him as he can get assistance at the right time so that he can improve his skills and can become independent. This funding assistance can help him to save his money for other works or personal works. By availing such benefits or assistance, his family are now able to manage his needs and this funding can even help to save his parent's savings(Scalioni and et. al., 2018). REFLECTION: Reflection here includes by the above care plan assessment is that as a social care provider, I empower myself to continuously enhance my work or the quality of care which I am providing to the patient. Reflection here allows me to share information with others also to increase or benefit their practices as well as mine. This can help me to perfect my practice so that i can enhance my relationships with others. A brief explanation of the whole care plan is described below with the help of the Gibbs model. It is one of the most famous cyclical models of reflection containing several stages for describing my experience. The stages include: Description:As aSocial worker, I worked for a patient called Ola Adeyemi who is suffering from Down Syndrome. I made a care plan such as assessment, planning, interventions and evaluation so that I can effectively provide quality care to Ola. I did all the tasks which includes providing the speech therapies, education and learnings, improving the communication skills, developing the awareness about patient care which Ola needs for his independence and improving his well being. In this, the only aim of my is to achieve all the goals which can lead to the betterment of Ola's health. Here my role as a social worker is to effectively execute all these interventions and planning that are related to the care plan like I have arranged a speech therapist and physical therapists, technical assistance who can work freely for such disabled patients. Feelings:During this process of a care plan, i feels that to succeeding the goals, it is very essential to manage the work which is between me and patient. This management of work is quite important for the patients and make them live independently as well as the quality of life. 6
Here, I am feeling gratified that i can completely achieve all these aims by choosing the correct care plan for Ola Adeyemi(Shields and et. al., 2018). Evaluation:from the above care plan, i experiences the effectiveness of the APIE framework which I implemented in Ola's case. Here i explains about valuing people plan, the therapeutic relationships between my and patient and the social model of disability effectively so that i can help Ola to accomplish all his goals which are required to improve his health. Analysis:In the above case of Ola Adeyemi, these things make me to understand that every person who is born disabled can have the potential or right to live their life more independently. It also comes under consideration that such interventions or planning can help disabled patients to enhance their well being(Sun and et. al., 2021). Action plan:From the above care plan, my experience here is to observe patients need and support him in various aspects such as emotional, social, physical and so on. I effectively add these particular experiences or skills in my personal life also so that these things can improve my health outcome also. CONCLUSION It is concluded from the above research that Down Syndrome is a genetic disorder caused by the extra copy of chromosome number 21. Due to this syndrome, people have to suffer from various developmental issues, mental health issues. As these people cannot able to live their life independently. To make these people live their life more independently, the social care provider made a care plan so that people can make improvements in their well being (As in the case of Ola Adeyemi). For effective health outcomes, the social worker has developed a care plan by using the APIE framework which generally includes the assessment, planning, intervention and evaluation. From the above study, it is also concluded that the use of effective skillsisquite important to establish a stable relationship between patient and care provider. It is also concluded by the social worker about the importance of skills such as communication, documentation and so on by which she can achieve positive outcomes in the case of Ola Adeyemi. 7
