INTRODUCTION
Day care services for health care patient is involving various facility this is the king of
holistic care facility. Under which service use get all type of services like social support,
refreshment and health care checks. Manager of the day centre have to take care of each and
every patient needs. If the user are showing any health care issues and symptom's then next step
is planing for the primary, medium and long term needs for care (Jennifer and et. al. 2019)
. Primary care is the initial step for the prevention of health issues by providing the health care
education and awareness about the health safety precautions. Secondary care is involving the
specialist focused care of the person due to their physical and psychological health requirement.
Long term care is involving the application of different variety of service's, which helps in the
management of medical and non medical needs of person. In this report care plan is going to be
develop according to the patient medical symptoms and requirement.
Assessment of person care needs with achievable goals by the aid of relevant structure, systems
and models and framework.
Health or person care need assessment is involving various step and practices which have
to be manage by the health care manager of the patient (Jarvis and et. al. 2021). Such as they
have to consider the medical history of the patient along with the analysis of patient strength
and weakness related to the ability of tolerating specific treatment. Manager have to make proper
analysis of evidence based practices related to the Brest cancer management and treatment.
Gretchen Hoffmeyer is 44 year old women with long term diagnosis of bipolar disorder, regular
medication is provided and taking the day care facility, recently it has been analysed that
Gretchen Hoffmeyer is having lump in her breast, which has diagnosed as malignant tumour. So
there is the option to have surgery to remove Breast, so that this can not effect other part of the
body. Below is the explanation if some heath care model which can be helpful for the effective
care plan development.
Out-of-pocket model- This is the type of care system under which only rich peoples can
get the effective health care and rest of the public life with their health care issues without any
care. There is know insurance and government plan. But in UK there is the best care models
and systems along with the appropriate government health care plan for poor peoples. Which
involves the effective guidelines for the peoples needs analysis and satisfaction (Haskins and et.

al. 2021). In context to the Gretchen Hoffmeyer health care and medical condition
management as a care coordinator, I had evaluated and applied health care model illustrated
below.
Holistic care model - This is the comprehensive model of patient care and management
, that is important model of the nursing science. The philosophy of this model is based on the
idea of holism . According to which for human holistic care is best rather then sum selective
parts of care. This model is involving the consideration of all area of care which can effects for
the outcomes. Such as medical, emotional, physical, and social care along with all essential
health care interventions and patient personal support services.
Needs assessment
As the care coordinator of Gretchen Hoffmeyer there is some important area of need of
assessment identified by me and explained below.
According to the NHSBPS there is the multiple steps of the Breast cancer patient needs
assessment and treatment. In context to the Gretchen Hoffmeyer, in order to accomplish proper
diagnosis to make effective care plan. There is the need for triple assessment clinic. Which
further discussed in brief by multidisciplinary team meeting in order to formulate management
strategy. In the Gretchen Hoffmeyer case surgery is suggested as the first line of control and
management. Because in most of the case Breast cancer surgery is possible and safe. There is
further needs of patient diagnosis related to their strength of managing treatment. According to
the R. H. Jack (2016) it has been analysed that with the patient who is having the mental
disorder and breast cancer is at higher risk of the death after the cancer treatment. After the
primary care referral patient have to go through from NHSBSP referral screening in NSH Brest
screening, then patient need urgent GP referrals to the diagnostic breast clinic. Where fast
delivery of the diagnostic and treatment services is provided to the patient according to the
national guideline (Fond and et. al. 2020). Then another services which should be provided to
the patients Brest screening and triple assessment. Under this process breast lesions are assessed
by the clinical examination image. Patient requires to go in close coordination with the surgeon,
nurses, physician, radiologist. Then next step is the multidisciplinary team meeting which is the
most important part of the breast cancer care pathway (Palacios and et. al. 2019). This facilitate
the improved quality of care for the patient. In this process case of the patient is discussed and
