INTRODUCTION
Meeting the needs of service user is the best concepts in which the organization work for
the social work and nursing for fulfil the requirement of patients and clients. This could be also
acquire with having the health care and needs to formulate the system for which they are
working. The main aim for report is to access the Ola Adeyemi’scare plan in which will support
hum and help for achieving the goals. This could also help for exploring the skills and also try to
include the theories and various approaches for making clear communicate about the given
structure and some of the good practices. This could also help for delivering the social worker
and try to ensure about how the disciplinary team should be required. Along with this also
examine about various models an practices which could acquire the implementing care to meet
the Ola Adeyemi’s needs.

The key standards about how usually the mental health care keep their good practice for
working towards the patients needs. While this could keep the multidisciplinary teams through
which this could be combined with the expertise and make sure for having the high range of
health care is used for deliver seamless and comprehensive care for the individuals. Ola
Adeyemi’s attend the seminar in which he come to know about how the good practices for the
social worker can be required and make sure for having some better policies with the
arrangements in keeping with what about the Ola Adeyemi’s wants for basically achieve in life.
As various problem that could be faced by him and this could involve the proper solution for
them through which they can easily come to know about how usually this could act for them.
Some of the better condition and role for social worker needs that could be trust relationship with
team, a shared vision of integrated care and make sure about their achieving team goals. Along
with this also keep some more about how usually this could act and strong system and keep the
team relationship accompanied by consistent working practices and some of the protocol.

The care plan formulation will enable to achieve goals significantly, for further enhanced
working targets for improvising Ola care needs to further ensure rest. Care plan will
significantly focus on specific needs in physical and mental wellness for improvising
present condition of Ola and to further assist in proper treatment. Furthermore, it has
been also found out that Ola faces issues such as improper physical condition and diet
issues which has mad him less energetic also and unable to conduct proper care of his
own. It has been also found out that Ola with Down Syndrome faces communicative and
physical issues with improper digestive system also (Hasaart and et.al., 2020).
First step is assessment, where it has been found that Ola , thirty year old man has Down
syndrome and has been facing physical issues from long time. Ola has attended school till
age of 18, parents were taking first care of ola but due to their increase in struggle to meet
previous needs significant recent care needs are unable to meet. Furthermore, it has been
analysed that Ola faces digestive issues and physical deformities due to Down syndrome
where he further is also weak in conveying proper communication. Ola has been also
significantly facing less consciousness many times, to properly perform his daily needs
and work on towards enhanced functional parameters(Kong, X. F., and et.al., 2020),
It has been assessed that Ola need medication , proper nutritious care and
therapies to further be proper and better in physical health for continuing his daily
activities. Ola faces depression , anxiety issuesw within behaviour which has impacted
proper wellbeing and is not able to spend time with same age people . Ola faces lack of
energy and slow physical and mental reactions have been found, which shows that there
has to be proper care taken of his nutrition for further enhancing physical strength. It has
been also analysed that medical care, nursing strategies and therapies need to be
conducted for taking proper care of Ola and to further take medical assistance will be
integral. There is infection found in Ola caused due to Down syndrome where it has been
causing lack of energy in individual making him typically unwell to be independent.
Down syndrome symptoms in Ola has been found that heart condition problem has
further led to hearing and vision issues within health, and also there are various
significant changes emerged in him (Moreau and et.al.,2020)
Second step is diagnosis, where Amniocentesis test, chronic villus sampling will be
further conducted for primary procedures and gaining results from diagnostic testing. It has been
