1) Response to the first question:
Informed consent in mental health such as provides individuals the adequate amount of
information regarding particular mental health issues so that individuals can take logical decision
regarding specific mental health issues.
Information such as essential risk factors about particular health issues, consequences of
operating problems and few protocols of operating issues shared with individuals.
According to laws of federal regulations of mental health, personal information about
patient’s mental illness such as depression should not be shared with individuals and in
particular, cases if shared then with the proper safeguard.
In case of informed consent in mental health, sometimes the patient is unable to permit due to
mental issues; in that case, it should be examined unlike other health services
(Grady, 2015)
2) Response to second question:
Current recordkeeping approaches that massively used are online record keeping and
using medical coding. In case of medical coding by using numeric digit histories and
epidemiology of particular mental illness can be obtained without revealing personal
information.
Steps to ensure the client access and pricy:
 Use of password to open operating system
 Backup of relevant data of patients
 Use of antivirus in computer
 Use of password protected email files (Hollis, et. al, 2015)

Third parties have access to the consumer's data via online sources of clinical record or
computer without password protection, or the third party might be a close relative of the
customer.
3) Response to the third question:
Diagnostic and Statistical Manual provide the familiar languages and standard criteria for
the classification of mental health with drugs that can use for curing.
It is essential to explain terminology to stakeholders to confirm that every individual will get
proper treatment. Besides, it will provide stakeholders to develop their plan for treatment.
Medical coding for quantifying patient is used international Classification of Diseases,
10th Revision, Clinical Modification (ICD-10) (Daniel & Makary, 2016).
The primary purpose of using this medical coding is that a three to seven alphanumeric
codes generated to indicate the signs and symptoms of specific disease in particular patient.
4) Response to fourth question:
Two best practices in communicating treatment of constituents to stakeholders within a
community health care setting are - physician explains the disease regarding taxonomy and other
potential side effects through verbal communication. Another one is the response to the emotions
of a patient with a proper protocol of treatment.
Multidisciplinary teams are essential in this organization because they examine and
diagnosis every part of the patient and suggest the best treatment care plan for each without any
significant consequences ( Murphy, Curtis & McCloughen, 2016). Every part of the team
examines the subject then possible best treatment care should be provided.
