Question Answers – Ensure each answer contains
relevant literature and relates back to the case
study.
1a) Using the case study provided, identify
an urgent risk area. Ensure your answer
details why you have identified this risk area
including specific information about the
client and current literature. (2 marks)
The most urgent risk of concern in Chung’s case is
his suicidal thoughts. Chung has been experiencing
fleeting thoughts of suicide for the past one week
and the fact that he is aware of highly lethal
medications that can help him commit suicide
make it worse. He feels helpless and hopeless and
wants to die. He even states that medication
cannot help him at all since his situation is self-
imposed. He is a doctor and therefore clearly
knows the most harmful medications he can
overdose. Moreover, males with suicidal thoughts
usually succeed in committing suicide as compare
to females (Klonsky, May, & Saffer, 2016)
1b) Identify one nursing / midwifery
intervention you would undertake directly
with your client to address the risk area
noted in question 1a and include a rationale
for the intervention. (2 marks for the
intervention and 2 marks for the rationale)
Ensure the intervention includes how, who
and when you would actually carry out the
intervention. Your rationale should state
why you would carry out the particular
intervention. Ensure literature is included.
1a and 1b – 200 words in total
As a nurse, I would start to establish a strong
therapeutic relationship with Chung. Since it’s an
urgent concern, this may start during the first
contact. To develop such a relationship, the first
step should involve listening and being empathic
for the emotional pain of the client so as to let him
disclose more information openly (Rosenberg, &
Chopra, 2015). Building a positive therapeutic
relationship with Chung is important since it will
help in attainment of successful assessment of
suicide and treatment that is effective. It will also
make it easier to develop working plans that will
promote the client’s safety (Butcher, Bulechek,
Dochterman, & Wagner, 2018).

2a) Using the case study provided, identify a
mental health concern. Ensure your answer
details why you have identified this concern
including specific information about the
client and current literature. (2 marks)
Chung has been feeling very low in mood for the
previous six weeks. He has experience
sleeplessness and particular insomnia with
wakening at 3 am. Moreover, he has had increasing
anxiety for the past two months. He now feels
worthless, helpless and is staring to have suicidal
ideation. This is affecting him adversely because he
even gave drug error in his work place and has lost
appetite which results in his weight loss. Low mood
in males is usually worse than in females since
males will not easily show it like females do and
therefore may go unnoticed for long (Oliffe et.al.
2017).

Psychosocial therapy will be the most appropriate
intervention as a nurse to undertake to help Chung
cope up with his low mood. This will involve letting
Chung open up concerning what is affecting him
and how he is feeling then reassuring him that all
will be well and should not worry. Reassuring will
also help in reliving his anxiety (Gilbody, 2016). This
will also involve counselling and motivational
enhancement including psychotherapy. A one on
one talk with his family will also be helpful since
they are the one to take care of him. This will help
Chung start to have sense of self-worth and even
feel better concerning his situation.
