Question 1
Scenario: You are asked to formulate a Risk Assessment for a site visit to a Gold Mine by a group of
visiting dignitaries and politicians. Base this on your visit to Kirkland Lakes Gold Mine at Fosterville,
Victoria. You will need to include the following in your assessment:
a) What was the most dangerous activities that the group may encounter within their tour?
The tour starts from when the group enters the Mine Lease Area.
The most dangerous activities which the group identified during the trip and which had greatest
impacts include (Fu et al. 2012 p.427).
i. Access tracks clearing and drill pads
ii. Construction as well as temporary use of the watercourse crossings
iii. Drilling
iv. Fuel, oil and chemical storage as well as transfers ( power, rail and gas)
v. Waste handling and driving in and out of the site
b) Identify each of the major hazards likely to be encountered, identify possible control
measures that are already implemented or could be implemented in the future. Using the
hierarchy of controls rate your controls from 1 to 5, where 1 represents personal
protective equipment and 5 represents elimination of the hazard.
The risk assessment has been identified in line with the aspects or hazards such as the noise, , heat,
pressure changes, segmental vibration as well as ionizing radiation . Also, the ranking of these risks
has been conducted via the appraisal of the potential impacts and consequences. This analogy has
been carried out via the utilization of the AS/NZS ISO 31000:2009 Risk management – Principles and
guidelines. The scale of 1 to 5 regarding both the probability and consequences has been used in the
ranking of the risks. This gives the likelihood of the occurrence of the risks in the long run. The
analysis largely illustrated as follows (Crosby 2012).

Further to this, the consequences regarding the environmental risks and assessments mainly tabulated

c) Do research and summarise what the regulatory bodies e.g. WorkSafe Victoria require
and what some of the major pieces of legislation a Gold Mine is legally bound to conform
to in terms of its Work Health and Safety (WH&S) obligations.
Some of the key requirements which the regulatory bodies deal with largely summarized as follows
(Leader, Robinson and Wilson 2012 p.384).
To deal with the body stresses, musculoskeletal disorders as well as manual handling
Falls and slips trips
Hitting which mayf results from the moving objects and machinery
Working with overall high risks machinery

As a mining consultant for a major investment institution, you are asked to provide a detailed
geological summary of the Fosterville Gold deposit. This review must include the following:
An overview of the regional geology i.e. the Bendigo Structural Zone in the Lachlan Fold
Belt.
The Bendigo structural zone has been distributed into three main faults which is naturally reffered
to as bounded structural zones. These zone include; the Melbourne zone, Bendigo zone as well as
the Stawell zone. The figure below is pictorial diagram showing the three zones that exists
