INTRODUCTION
Health care is the term which defines the person’s health and how to take care of it in an
effective way (Rotter and et.al., (2019).
The report will discussthe value of collaborative partnership model of care when working
with the person and their family. Along with this, developmental stages of person with
determinants of health will be discussed to be considered prior to changes in behavior. And
finally, the process of motivational interviewing will be explained for empowering the person to
move forward towards self management and their chronic condition. Therefore, this made sure
that these aspects for disease were to be considered in identifying main problems and how to
solve problem on large and small scale.

Discuss value of using a collaborative partnership model of care when working with the person
and their family
Collaborative partnership model of care is the model which helps in collaborating with
patients and their families. Collaborative model helps in taking care of patients needs and
requirements and analyzing what and how the medical services are to be provided to the patients
who are need. The health care is factor which is observed through collaborative approach which
included the involvement of many health care professionals and the patients who are in need of
the services. There is partnership between patients, providers, and their families for taking
decisions, coordinating with patients and their families (Nyström and et.al., (2018). Collaborative
partnership model of care is very important in health care industry and for patients and its family
members as well.
In current case study, nurse can use this model to work with Ron. Here, nurse can involve
Ron wife and his relatives as well friends in treating Ron. Also, the nurse can take decision by
involving Ron as well. Moreover, nurse can involve other health professionals such as
psychiatrists, social workers, etc. in order to treat Ron. By interacting with Ron wife, nurse can
find out Ron needs and accordingly take decisions. Thus, in this way by working in partnership
nurse can treat Ron. In this case, Ron will get help in overcoming and recovering from the
disease of Parkinson as his wife is concerning to take care of him with help of community RN
so that she can understand her husband well and is able to communicate with him (Shihabuddin
