Background:
According to Moore (2014), pressure ulcers or bed sores are skin injuries that are
caused on account of prolong pressure exerted on the skin surface. It should be noted in this
context that pressure ulcers develop on the areas of skin that cover bony surfaces such as the
heels, ankles, tailbone and the hip. As stated by Efteli and Gunes (2013), patients who remain
restricted to bed or chair on account of a limiting illness are most susceptible to develop
pressure ulcers. With proper management of symptoms and use of appropriate intervention
strategies, bedsores can be managed effectively. Research studies indicate that 22% of the
patients within the hospital are affected with pressure ulcers (Cooper et al., 2013). This
assignment intends to scan the available scholarly literatures to access the risk factors that
cause pressure ulcer and present a discussion on the treatment measures available.
Research Rationale:
The rationale for conducting the research can be explained as the evaluation of the
scholarly literatures to discuss measures that could help in the treatment of pressure ulcers. In
addition to this, the research would also help in the implementation of best evidence based
practice in order to prevent the risk factors that trigger the manifestation of pressure ulcers in
hospitalized patients.
Research Hypothesis:
The research hypothesis for this research study can be mentioned as;
“Evaluation of the risk factors that trigger the formation of pressure ulcer and the available
evidence based practice measures can help in the prevention of the cause and progression of
the pressure ulcer in the patients.”
Research Objectives:

On the basis of the research hypothesis, the following research objectives can be
summarized:
To evaluate the risk factors that lead to the formation of pressure ulcer
To analyse the evidence base to identify the best treatment available for the treatment
of pressure ulcer
Identification of other discipline connected to the research activity:
It should be critically noted in this context that formation of pressure ulcers within the
heel or the ankle region limits the mobilising ability of the patient to move from one place to
another. In order to assist with movement, the inclusion of a physiotherapist and an
occupational therapist can be extremely useful. The rationale for the same being assistance
with mild physical exercises and energy saving technique which would help the patient with
movement and localization.
Search Strategy:
In order to scan the evidence base, a thorough search was conducted across the
electronic databases that included the Google Scholar and Cochrane Library database. A
number of research papers were retrieved that were filtered on the basis of the inclusion and
exclusion criteria and were considered for the research.

Methodology:
Data collection process:
The methodology that would be followed to retrieve information from the scholarly
evidences would include, conducting a literature review based on the use of appropriate key
words and conducting an exhaustive systematic review of the retrieved literatures. The
rationale for the selection of the methods can be mentioned as less time consuming. Further,
building on the available evidence base would help in developing a clear idea about the
research done so far and would help in identifying the existing research gaps so as to work on
those areas.
Discussion and evaluation of collected data:
According to Moore (2014), use of an appropriate pressure ulcer risk assessment tool
or a scale could help in identifying individuals that are at a high risk of developing pressure
ulcers. In addition to this, the use of a pressure risk assessment is also recommended by a
number of pressure ulcer prevention International guidelines. The researchers conducted a
review to evaluate the patient outcome associated with risk assessment of pressure ulcers
within clinical setting. The overall findings revealed no statistically significant results in the
quality of patient outcome for nurses who used the Braden risk assessment tool. However,
results showed improved patient outcome in nurses who made use of unstructured risk
assessment. The limitation can however be mentioned in terms of the studies considered for
the review. According to Cooper et al. (2013), the use of Norden, Braden, Waterlow and
Jackson-Cubbin scales would help in determining the risks of developing pressure ulcers
within the critical care unit. Further, the paper also mentioned that the use of device-related
pressure ulcers could help in prevention of pressure ulcers in patient. However,Efteli and
Gunes (2013) mentioned that the consideration of the patient’s gender and serum albumin
