Abstract
The thesis of the paper presented entails a nursing case study detailing out some of the possible
health status of Mr. C. Mr. C is seeking medical information based on his health status that is
information based on his obese nature hence he believes that bariatric surgery will help control
his obese condition.
The paper further explains various medical diagnosis and intervention based on the symptoms he
has been experienced for the past few months. The potential and actual health problem for Mr. C
from the diagnosis conducted to Mr. C to aid in managing his health condition.

Potential diagnosis and intervention
The clinical manifestation diseases for Mr. C thus according to the case study are the
diseases which can be determined through talking to patients and physical examination of the
patient. To Mr. C’s case, one of the clinical manifestations which can be observed by the nurse
through physical examination and talking to him is overweight obesity due to his body size and
high blood pressure (Ma, He & MacGregor, (2015).
The potential risk factors for the overweight obesity according to Mr. C’ medical history can
be identified as the body inactivity thus lack of physical exercise and genetics. According to the
case study, Mr. C explained that he is always seated in his place of work as he works as a
telephone center. The issue of body inactivity has predisposed him to obesity condition since the
body lacks proper metabolism which helps to burn excess fats out of the body (Baidal, et al.,
2016).
The issue of genetics according to the Mr. c’s case study is witnessed by explaining that he
has been overweight since his childhood up to date and he wants to do away with the condition
and that’s why he is seeking information on the bariatric surgery for his obese condition.
Genetics factors came in when one of his parents were obese hence they could pass such traits to
him.
Bariatric surgery includes a series of procedures performed on the obese people by reducing
the size of the stomach with a gastric band or through the removal of a portion of the stomach or
by re-routing section of small intestines into a small stomach pouch. According to Mr. C’s
medical profile that is overall weight and age, he best fit for the bariatric surgery since he weighs
over 100 pounds and he is between 17-70 years of age. Therefore, the doctor should recommend
him for bariatric surgery (Simopoulos, 2016).

Functional health pattern
From the case study presented based on Mr. C’s medical history, both actual and potential
health problem has been identified based on his functional medical patterns. The problems are
discussed below in the context by identifying the associated rationale for the potential and actual
health problems.
One potential health problem to Mr. C is the diabetes condition. The main risk factors for
diabetes conditions are overweight obesity. According to the case study, Mr. C has been obese
for long since childhood hence one of the potential health problems he might suffer from is
diabetic.
Due to Mr. C’s high blood pressure condition, the potential health problem he is likely to
suffer from is the coronary artery disease. High blood pressure condition is a major risk factor
for coronary artery disease since they are involved in blood circulation as they can either burst
due to high blood pressure.
The actual problem according to Mr. C’s medical examination has been identified as the
overweight obesity. This is according to Mr. C’s medical examination results.
Another potential health problem Mr. C might be exposed to according his explanation is
congestive heart failure. This is possible due to swollen legs and ankles. The condition can also
cause shortness of breath.
Potential health problem Mr. C can be due to nerve disorder. This problem may be due to
itching skin.

ESRD prevention and health promotion
ESRD is the last stage of kidney failure and it can be prevented by adhering to the following
procedures to prevent further damage to the kidney. Mr. C should control his blood sugar and
keep a healthy blood pressure to prevent further damage of the kidney infected by the ESRD
condition since these are the triggering factors for the condition. According to his health case
study, he should keep a healthy weight since his overweight obese condition will worsen the
ESRD condition (Cobb, et al., 2018).
Health promotion for ESRD patients is considered to be the desired objective of dialysis
treatment. Dialysis treatment will help to improve the functioning of the kidney and the life of
the patient since it will enable removal of waste harmful chemicals from the body by artificially
using of machines (O’Shaughnessy, et al., 2015).
Health education which will be provided to Mr. C on his health status that is how to prevent
future complications based on his medical status that is ESRD is control of salt consumption,
regular checking of his weight and blood sugar level (Yap, et al., 2015).
Resources which are available for the ESRD patients provided by the government and other
non-governmental organizations include housing grants and dialysis medical cover since most
the ESRD patients are not able to pay for dialysis due to their financial status and high cost of the
dialysis process.
