In this assignment, I will be discussing the case study of Mrs. Perkins, who is 77 year old
aboriginal woman and is suffering from multiple healthcare conditions. During her hospital stay
and after being discharged to home, I provided care to Mrs. Perkins and hence, using the Gibb’s
reflection cycle I will be explaining all the six the six steps of this reflective cycle aligning it to
the care process for Mrs. Perkins. Further, in that course, I will be explaining the barriers and
enablers of the care process that hindered or assisted the patient to change their health and
wellbeing.
Gibbs reflective model is the theoretical model using which nursing students are able to
focus on their course learnings and reflect on the positive and negative aspect of it so that they
can understand their strengths and weaknesses while going through the learning (Husebø,
O'Regan & Nestel, 2015). the steps of the Gibbs reflection includes description of the situation,
feelings, evaluation of the situation, analysis, conclusion and a detailed action plan used to
compensate the negative aspect of the situation (Howatson-Jones, 2016). I will be applying these
six steps in this reflective assignment.
Description
While caring for Mrs. Perkins, who is a 77 year old aboriginal woman and was suffering
from chronic urinary tract infections (UTI’s), chronic kidney disease (CKD), sleep apnoea,
osteoporosis and deteriorating mobility and several other diseases. When I went to the ward of
the patient, I saw her unsettled and uneasy on her bed. However, her handover report mentioned
that she was responsive and with slightly increased blood pressure, normal body temperature,
slightly increased respiratory rate, and pain score relatively low. On the other hand, when she
was described to her home, she was cared properly however after the death of her mother, she

started feeling that she is unable to take care of her health condition. Therefore, these two
scenario were important for the care process of Mrs. Perkins and my focus for refection will be
these two scenario.
Feelings
While caring for the Mrs. Perkins and her ailments, the first thing which I was concerned
about her being an aboriginal and making the care process culturally competent. Besides these,
the health condition of patient was deteriorating and her diarrhea related condition was an issue
that I had to resolve so that the patient could be provided with comfort. Hence, I started feeling
the needs of prioritizing the care process for Mrs. Perkins and hence, I felt that my responsibility
for the patient increased as I was responsible for her health condition for my duration of shift.
Hence, I was cautious of every intervention I was applying in the care process. On the other
hand, after the patient discharged to home and was receiving care and intervention there, I felt
that the family of the patient was supportive and readily took responsibility of the care process.
Evaluation
While evaluating the care process, I was able to determine the good and bad aspects of
the care process that was carried out for Mrs. Perkins. The good aspect for the care process was
the communication process and family centered care that was applied in care of this patient
(Inzucchi et al., 2015). The family of Mrs. Perkins was very supportive and after the discharge,
they were completely involved in the care process. They used to take the patient in the healthcare
facility for each appointments, medication administration and diet and nutrition related sessions.
Further, they were also involved in the communication process and conveyed every important
