1. Identify and explain each step of the Clinical Reasoning Cycle in your own words. Your
explanation should include examples of the types of activities involved with each step.
The clinical reasoning cycle refers to a process where medical practitioners and nurses
collect indications and process the data and find out the problem. There is planning and
instrument interventions reflect whole process. There are different steps of clinical reasoning
cycle which are going to be discussed further.
Considering patient situation- This is the first step in which there is collection of
detail about the patient. This includes various situation and information of the patient. Mrs.
Roberts is 60 year old woman who is having hip replacement surgery and having high blood
pressure. This information should be consider during providing surgery of the patient. In this
step, there is consideration of every situation of patient which can provide better information
related with pre-surgery. There is evaluation of her normal blood pressure, heart rate,
respiratory rate, body temperature and other necessary procedure to follow for providing
better situation for Mrs. Roberts (O'Connor, (2018)).
Collecting cues- This is the step in which there is collection of information like report,
any previous medication history of patient, nursing assessment or other data related with
patient that is collected by nurse. After analyzing the data and information that is collected
are have the recall like epidemiology, therapeutics etc. Student nurse has collected all the
information by using questionnaire which provide information related with Mrs. Roberts.

of the problem of Mrs. Roberts. Here. Nursing staff need to identify health related problem
which can have impact on Mrs Roberts hip replacement surgery. It is essential to identify
issues appropriately, then this surgery can be implemented. Nursing staff need to identify the
problem which can be related with surgery where Mrs Roberts should not be comfortable and
can have negative health outcome.
Establishing goals- Nurses have the role of replacement of hip within director time. In
this, the goal is clear and nursing staff can work towards it and conduct the hip replacement
surgery. Nursing staff have the goal for providing successful hip replacement surgery of Mrs.
Roberts. Here, nursing staff is having the goal of successful hip replacement surgery which
should not having any type of abnormality observation after the replacement surgery. There

Taking appropriate actions- As per the collected information, nursing staff have the
role to provide better outcome by offering their services in desirable actions. In this, nursing
staff need to take care that they should not insert penicillin injection to Mrs Roberts. Nursing
staff need to take appropriate action and should consider that there is no use of penicillin due
to causing allergy in Mrs. Roberts.
Evaluating- Role of nurse is not only the procedure of caring of patient. They are also
concerned with effective outcome which has been achieved by different actions. There is
need to guide student nurse for evaluating parameters which has been provided by them. This
can help for revaluation process of Mrs. Roberts after successful hip replacement surgery.
There is a need for evaluation of surgery which has been done successfully and should
evaluate the outcome for considering normal health of Mrs. Roberts (Loh, (2017)).
Reflecting upon the process- This is the last and final step which is involved in this
process. There is need to reflect upon the learning which has nursing student and nursing staff
has been gained through this. It also provide the information and collected after performing
actions during hip replacement surgery about the reassurance about hygiene maintenance and
providing care to Mrs Roberts. Student nurse has also learn about collection of each and
every information which should be directly or indirectly related with surgery. This can lead to
provide better outcome and can have better response after surgery.

in the procedure for better performing the surgery for the better outcomes. This is able to

Mrs. Roberts have normal heart rate, respiratory rate, oxygen saturation is also normal
which should be in between 95% - 100%. Here, Mrs. Roberts is having 99% of oxygen

saturation in her body. This shows the normal finding through health assessment (Peer,
(2017)).
Mrs. Roberts have history of high blood pressure and feels dizziness sometimes.
Sometimes she feels of funny rhythm of her heartbeat. She feels little lightheaded which she
need to overcome by lapping over by relaxing. She is not able to get penicillin which is due
to having side effect on her body which in form of rashes all over the body and her face gets
swell up. She consumes some medicines for managing her high blood pressure that is Betaloc
and Warfarin.
It has been find out by the observation and collected data by health assessment before
the hip replacement surgery which is for better information and analysis of her health which
should be healthy for executing surgery. It has been find out little bit deviation in her blood
pressure and there is no any evaluation of other health related issues. There is abnormality
that Mrs. Roberts have high blood pressure and sometimes absorbs funny rhythm during heart
beat (Hakim, (2017)).
Abnormal findings
There is health assessment of Mrs. Roberts. There is little bit deviation in the diastolic
blood pressure which shows the abnormalities in the observation. There is also abnormalities
of use of penicillin which show allergy or side effect after use.
Mrs. Roberts has provided the information that she is suffering form the high blood
pressure. It has been also informed that use of penicillin creates adverse effect which leads to
create dangerously rashes on all over the body and swelling of face is observed which is
abnormal and need to be avoided the use of Penicillin on Mrs. Roberts. She is not feeling
good during assessment with Student nurse about different questionnaire which was there for
proper findings of information for evaluation and for implementing surgery of hip
replacement (Coventry, (2017)).
As per the health assessment, it has been found that health assessment it has been
found that there is deviation in the diastolic blood pressure which provides the information
about there is low coronary artery pressure which can lead to create lack of blood and oxygen
in heart can be observed.
