INTRODUCTION
Nursing Interventions are commonly known as actions which are undertaken by the nurse
for the better care plan of the patients including the treatments, procedures or teaching the patient
to improve their comfort and health areas. The present report will illustrate the nursing
intervention of two selected disease which are Fall risk and Delirium. These two clinical issues
provide the difficulty in patient treatment and care (Kunz and Minder, 2020). In this report, the
nursing intervention in the care of people with the complex health care needs which be discussed
and mentioned for the better results.
MAIN BODY
Nursing Intervention for Fall Risk
The risk of falling occurs mainly with the older group of people where they become weak
and loses their energy very quickly (Pratiwi, Hamid and Fadhillah, 2018). The nurses who are
taking care of the people or a patient have to adopt some nursing intervention like regular
exercises and physical therapy programs which can help the older people to improve their diet
and strength effectively (Fakoya, McCorry and Donnelly, 2020). The nurses are also required to
take care of the patient by cleaning up the wet floors from where it can avoid the falling and
slipping on the floor. And, if required also focus on the mobility plans for the betterment of the
patient.
Nursing Intervention for Delirium
Delirium is a mental disability which results in confused thinking and reduces the awareness
of the surroundings. It is a kind of disease and clinical problem which require the special
treatment and care plan for the betterment of the patient health (Rubensson and Salzmann
Erikson, 2019). Nurses have to understand and identify the level of anxiety of the patient
suffering from the delirium and should watch their behaviours so that before any violence the
nurse can be able take care of the patient effectively. Also, it is a duty of a nurse to provide the
good and positive environment to the patient which will enable the patient to feel good and low
level of anxiety (Sloane, Smith, McHugh and Aiken, 2018). A proper assistance is needed and
required by the nurse to take care of the delirium suffering patient as it can be dangerous to leave
them alone as they are disoriented, confused and can harm self and other as well. Nurses should
be polite and kind where the patient can be able to speak their problem and issues to the nurse
and they also should respect the right and dignity of the patient to be treated well.

CONCLUSION
The report has concluded the meaning of the nursing intervention and its importance which
is required to be considered for the better treatment and care of the patient. It has included the
two clinical issues which are required the nursing interventions to be implementing and definite
manner so that the older age group of people can be able to get the accurate treatment and care in
effective way.
