Assessment 3: SMART GOAL: Analyse and identify task duplication when completing
admission process in Acute Adolescent Unit during admission process from XX number of
patient by June 30th 2021
Introduction
This paper will report a situation analysis to inform a strategic plan to explore time wasting
duplications in the admission process. In my clinical area several tasks are undertaken by
Registered Nurses during the admission of young people to an adolescent mental health. The
young people are often agitated and or distressed about the admissions, asking lots of
questions and or making complaints. Accompanying parents and or carers may also be upset
about what is happening. Sometimes there is tension and a confusing context for the
admission process. Nurses undertake head-to-toe assessment, collect important information
and document findings during the admission process. The tasks at present take approximately
20 minutes. There is concern and curiosity about the process and that some tasks may be
duplicated and some perhaps obsolete. History and culture of doing things the same way
without regular evaluation may have led to some duplication and less efficiencies in the
admission process. In this context task duplication is become one of the common issues
during admission process.
Situation Analysis
As per concern of task duplication most of patient get disturbed as it takes even more time to
analyse appropriate information about the illness of patients. For this possibility to measure
and control all the relative information about patient is having negative impact. In this most
of the patients are not much supported as it induces balanced ciaos that might result into
diverse impact over health care of patient.
Task duplication induce potential impact over individual health as with this possibility
to provide safer and free care to patients is addressed by health care service providers with

better relevancy. In order to reduce the impact more of nurses get spend perpetual time for
better recovery of patients and along with this invest appropriate time to reviewing all the
relevant changes through which delegable changes is implemented in formulated manner.
My desired outcome from this goal is to reduce task duplication when completing admission
papers and improve time efficiencies. There is concern that errors are made during the
process which may be identified in the course of this work.
Data and Benchmarking
while minimizing errors for registered nurses in Acute Adolescent Unit during admission
process by June 30th, 2021.
To attain adequate benchmarking, smart goals is developed that is followed by nurses
to offer practical measures to patients as with this rate of duplicity get reduced.
Stakeholder Consultation:
Of the staff consulted most considered that suitable programs are conducted as per
removal and reduction of admission process as with this possibility to recognise sustained
practical setting is processed within timely frame. There are various improvised diagnostic
tool and techniques are used that support clinical support that capture all the required
information which directly facilitate exchange rate of information.

There is various risk that is associated with task duplication as with this potential of error is
developed and processed that is riskier for the patient and for the health care service provider.
It also takes a higher amount of time with this possibility to manage as well as measure
various risk credibility’s is attained in timely constrained.

Combined Analysis Findings
For this gap analysis also get analysed as with this adequate focus and concern is maintained
towards fulfilment of complex situation with clarity as in this desirable outcome is processed
in moderate basis. The current state of quite in negative prospect and that is due to task
duplication through which unbalanced working surround is developed that is not favourable
for staff working. In terms of conflict generation, it is required that future state and scenarios
is mapped with this service provider worked as a team through which rate of task duplication
is reduced in favourable manner. It also has direct impact over enhancement of healthcare
service that is provided to the people as with this working capability and its tendency both get
advanced on regular basis. For this wider level of service is developed that aid to develop
strong level of interrelation with the patients and in this trustable relation is developed as in
global context with this patient’s interaction is attained in effective basis that also promote
and improvise service standards in marketplace (Machon, Cundy & Case, (2019)).
To align and manage with weaknesses it is required that resources are utilised in effective
manner and with this task duplication also get controlled in favoured manner (Stanley &
Stanley, (2018)). The resultant of this, satisfactory changes are implemented that reduced that
level of compliance under which balanced outcome is achieved and for this service provider
control major prospect with this long term sustainability is attained within timely frame. In
this suitable level of opportunities are developed and in this higher manpower is implemented
as with this flexible distribution of service is induced on random basis.
Data and Benchmarking –There are a number of patient admitted in Redbank House acute
adolescent unit and preferably patient of aged in between 12-18 year. The house provided
suitable spanning of service that is suited to local, regional and state wide service which is
resultant of community integrated system of primary health care. It has been done by
advancing acute containment and stabilisation which is involved and drive recovery focused
approach.
Literature Review
Qualified nurses more often identify clinical error and reduce inevitable stages
throughout their career (Ford, 2021). Nevertheless, as per reduction of harmful outcome and
