INTRODUCTION Medication management is referred to interdisciplinary research teams which examined roleof nursingandcontributiontoprevention andmanagement ofmedicationerrors in transitional care settings and hospital(Dougherty & Lister, 2015). This report will include Marcel's medication management, nursing interventions and rationales. And also the strategies used for the nursing interventions. MAIN BODY 1. Priorities of Marcel's medication management Marcel is 70 years old who is suffering with diabetes, obesity and also medical history with hypertension. As Marcel reduced the physical activity and earlier Marcel was heavy smoker which affected the Marcel's body. There are some priorities of Marcel such has reducing weight which will help Marcel to lead a healthy life. Proper medication for the diabetes has to given which will decrease the severity of diabetes. Well managed life style like sugar, junk food and otherunhealthyfoodhastobeavoided.Hyperglycaemiaarisesfromillnesswiththe haemoglobinopathies. Maintaining blood glucose level lower the risk in patients who are using oral drugs which lower glucose(Xu & et. al., 2014). Marcel has to quit smoking which will help to reduce the higher risk of other diseases which are associated with it. Medication has to given to manage the weight and some self monitoring care plans which will help to lead a good lifestyle. 2. Nursing interventions and rationales used to support Marcel's medication self management Diet has to be maintained according to the nurse suggested and quantity and type of food also has to managed. For BMI therapy can be given which will help to reduce the weigh. Marcel has to involve in physical activity at least for 30 minutes per day. Smoking and alcohol consumption has to be decreased. Self monitoring like glucose is suggested for those who are with diabetes using insulin which can cause hypoglycaemia. Nursing interventions are very important for the diabetic patients it helps them to control the severity of disease(Tabloski, 2014). Marcel is provided with the proper care plan which help Marcel to control the diabetes and reduce the risk of hypertension which can increase with the diabetes. Diagnosis for the diabetes which will help to give treatment for diabetes. Type 2 diabetes cause overweight in 1
individuals and make them obsessed. It is also because there will be insufficient insulin in produced in body and underutilized insulin by the individual's body. 3. Strategies used to evaluate outcomes of nursing interventions Strategies like explicit goals setting with the patients and also supporting patients to manage the healthy lifestyle choices such as healthy eating, weight management, tobacco cessation, effective coping and physical activities. Preventions of diabetes complications and disease self management. These strategies will give good results in case of Marcel. Helping with theproperdietmanagementisrequiredforthenursinginterventions.Therearesome programmes which are make to aware people for the intake of tobacco and alcohol consumption. Behavioural strategies which include problem solving and goals setting which help the diabetic patients to control the condition(Vaismoradi & et. al., 2014). High quality of care should be given to diabetic patients and in case of Marcel required more care as being with obesity, diabetes andalso with the hypertension health history. Good quality of diabetes care involve basingcareontheevidence-basedguidelines,educatingandactivatingpatientsand implementing the care. These strategies will help to give proper medication to the Marcel and it will control diabetes condition as well as overweight. 4. Application of clinical reasoning cycle Clinical reasoning cycle is a process of the critical thinking as well as responses is not linear process. It include patient's situation then collecting information about the particular situation, issues or problems and actions has to be taken. Clinical reasoning cycle helps to patients as well as nurses to estimate the problems are care setting which can provide good outcomes(Adhikari & et. al., 2014). The nurses are trained to provide proper care and treatment to patients which help them to get a better care and control the disease. With the help of this cycle nurses and professionals were able to provide diagnosis and management of the diabetes. It helped professional to set some goals according to the health condition and give treatment considering all the heath condition. As clinical cycle include every type of stages related to the treatment or before that which gives an overview on the particular condition. Marcel's diabetes and obesity was able to control with the help of this cycle. 2
