INTRODUCTIONMedication management is referred to interdisciplinary research teams which examinedrole of nursing and contribution to prevention and management of medication errors intransitional care settings and hospital (Dougherty & Lister, 2015). This report will includeMarcel's medication management, nursing interventions and rationales. And also the strategiesused for the nursing interventions. MAIN BODY1. Priorities of Marcel's medication management Marcel is 70 years old who is suffering with diabetes, obesity and also medical historywith hypertension. As Marcel reduced the physical activity and earlier Marcel was heavy smokerwhich affected the Marcel's body. There are some priorities of Marcel such has reducing weightwhich will help Marcel to lead a healthy life. Proper medication for the diabetes has to givenwhich will decrease the severity of diabetes. Well managed life style like sugar, junk food andother unhealthy food has to be avoided. Hyperglycaemia arises from illness with thehaemoglobinopathies. Maintaining blood glucose level lower the risk in patients who are usingoral drugs which lower glucose (Xu & et. al., 2014). Marcel has to quit smoking which will helpto reduce the higher risk of other diseases which are associated with it. Medication has to givento manage the weight and some self monitoring care plans which will help to lead a goodlifestyle. 2. Nursing interventions and rationales used to support Marcel's medication self managementDiet has to be maintained according to the nurse suggested and quantity and type of foodalso has to managed. For BMI therapy can be given which will help to reduce the weigh. Marcelhas to involve in physical activity at least for 30 minutes per day. Smoking and alcoholconsumption has to be decreased. Self monitoring like glucose is suggested for those who arewith diabetes using insulin which can cause hypoglycaemia. Nursing interventions are veryimportant for the diabetic patients it helps them to control the severity of disease (Tabloski,2014). Marcel is provided with the proper care plan which help Marcel to control the diabetesand reduce the risk of hypertension which can increase with the diabetes.
