The features of the mental state examination (MSE) that is related with the case study
include:
1. Appearance and behaviour: The person who is suffering from depression shows
decreased interest in grooming, hygiene and cleanliness. However, Munny was well dressed
when he visited the centre. His clothes were loosely fitted. The person can appear tired and
older and this was again not relevant in relation to the case study. Psychomotor retardation
slows down the mental and physical activity. Thus the affected individuals move or talk
slowly (Fernando & Carter, 2016). Psychomotor retardation might be case of Munny as he
was refusing to communicate spontaneously. His wife reported that he only communicates in
monosyllables and in clinic was only responding nodding his head.
2. Mood and Affect: Low mood is a pervasively low with constant feelings of sadness
(Fernando & Carter, 2016). The client might feel hopeless or worthless or powerless to do
anything in his life. The constant feeling of sadness is evident in Munny as he is detached
himself from the social life. Munny is refusing to talk, eat and to spend time with the family
members. His feelings of worthlessness are evident as he is refusing to go to his professional
field and is constantly taking sick leave.
According to the Diagnostic and Statistical Manual or Mental disorder V (DSM V)
published by the American Psychiatric Association (APA) two major symptoms of
depression include depressed state of mind or loss of interest. Depressed state of mind
persists throughout the day and is indicated by subjective report or through observational
report made by others (Armour, Műllerová & Elhai, 2016). In case of Munny, it was
observational report. His wife reported that Munny mainly keep himself confined in his own

room refuses to communicate, take part in the household activities or spends time with his
children. The second symptom includes significantly diminished interest or pleasure in all
daily activities. Munny also refuses to do household activities as reported by his wife
(observational report). Significant weight-loss or weight gain is also evident among the
individuals who are suffering from depression along with decrease of appetite. Munny’s wife
reported that his has lost considerable amount of body weight during the last few weeks and
refuses to consume meal (indicating loss of appetite) (Armour, Műllerová & Elhai, 2016). In
the clinic, Munny’s clothes were loosely fitted indicating recent loss in weight. Psychomotor
agitation or retardation is another symptom in depression. In case of Munny, it is
psychomotor agitation as he refuses to talk with his wife and asks him to leave him alone in
the room. His depressed state of mind might be due to feelings of worthless or excessive
guilt. Munny’s boss has informed him that he is unhappy with his work and if does not
improves his skill, he might lose his job. This verdict from his boss might have generated the
feelings of worthlessness. Munny in the clinic has also approved that at times he things
whether this life is worth living. Recurrent thoughts of death or diminished ability to think or
concentrate are not relevant as her the symptoms of Munny highlighted in the case study
(Cooper, 2018).
Question 2
The stress-vulnerability model helps in identification and treating of the mental health
complications. According to the stress vulnerability model, the vulnerability of developing
depression and anxiety in the life of an individual increases with the increase in the level of
day-to-day stress. The increase in the level of stress can be occur through the life or can
increase rapidly at single point of time (Wasserman & Sokolowski, 2016). The stress event in
the life of Munny that has made him vulnerable to depression is highlighted below:
