Patient situation:
The case scenario is about Mrs. Violet Kwong, a 48 year old widow, who has been
referred to the community nursing team by her surgeon for assessment and management of a
small wound on her right lower leg. Six weeks ago, she had a melanoma removed from her right
lower leg. The vital sign assessment result overnight revealed respiratory rate of 16, blood
pressure of 100/60, heart rate of 115, temperature of 38.3 and presence of mild ketones during
ward analysis. Her social history revealed she has two children. Her son lives in Perth and
daughter gives her a weekly visit. However, currently she feels very isolated as she lives alone
and there is no body by her side to take care of her emotional and social health issues. The
presenting health issue for Mrs. Kwong is non-healing wound and mental distress due to
melanoma and social isolation.
