Betty's health problems have grown to be very critical and need assistance. She has a couple of
acute diseases as well as chronic conditions. According to her situation, and her family condition
basing on the fact that her husband is old and their daughter is near them, but she only visits
regularly, it's useful to advise the family and the patient to seek the best option to provide
adequate care for the patient.
Betty's Concerns
Betty got diagnosed with Chronic Kidney Disease (CKD). To add on that she has her previous
health condition has been poor she was diagnosed with Type 2 Diabetes, IHD, PVD, HT and a
STEMI as per past diagnosis history. Her concerns are that according to the situation of her
illness, and as per the doctor's words, she is not likely to get better, so she wants to discuss with
the team handling her medication. Betty thought of having a talk with Leanne who is a nurse
practitioner to know the way forward in case her sickness persists. Her renal needs should be
well integrated with care plans (Smith et al., 2015).
Goals of Care that Might get Identified following Betty's Discussion
Goals of care conversation experience so many barriers hence it's a hard task for medical
practitioners. The obstacles to the communication might be the family, patient or the facility as
well as system barriers (McLennan et al. 2015). The goals of care in Betty's relationship might
achieve goals such as her being taken to a multi-disciplinary approach in palliative care
(Nicholson et al., 2018). In her case, a family, provider and patient agreements and decision
making need to take place while taking into consideration the preferences as well as her future
medical plans. A multidisciplinary team will facilitate the care. The goals of care communication
will help Betty come up with a solution for how advanced care planning concerning her situation
