This project report is all about providing specific information about “Coroner recommends changes after blood mix-up patient death”. Analyzing of the incident is being effectively and related consequences to them. Comprehensive description According to the mentioned case, it has been seen that a senior patient who was 71 years of age died because she received a transfusion of wrong blood types. The cause of death is basically improper behaviour of the carer and relatives that to be encouraged to accompany heart patients to pre-surgery process. Prior to the surgery, “Ruth Stoll” was needed to go through Clinical-path laboratories to provide sample of their blood so that it could be tested during the time of transfusion. At the same time, another patient named “Martha Kovendy” has also present for the giving their sample. The nurse that took the samples mislabelled the two test tubes. During the transfusion process, she did required blood and received the wrong blood. Because of this, after six days later she died (ABC News, 2018). Explanation about the event on I feel and why it made feel After gone through entire case scenario, it has been establishing that various unethical dilemma or mistakes is being happen during the time of blood sample process. The one which is noticeable is the presence of relatives such as sister-in-law and Ms Kovendy’s husband remain outside in waiting premises while the blood samples were taken the patient. From my point of view, I found that the entire mistake is done from the nurse side. Because of their causal behaviour or not focused on their entire job, such kind of problems got arises. The relatives must be aware about the condition and all the tests that should be done with Stoll. They must be 3
always there with the patient to check all the situation on the personal perspectives.The casual behaviour of nurse has made me feel that how could she did such kind of mistakes. Because as a she must have checked after labelling the samples. This particular reason made her died. The main reason I think it made feel miserable for the clients because of irresponsible skills and casual performance of Nurse that has taken the blood. Because of her mistake Stoll get wrong blood that causes her death. Some mistakes are also from the relative or carer side because they are not present with her while taking the blood samples. Evaluation of all consequences of this critical incident for the health consumer There are various educational programs associated with NSQHS (National safety and Quality Health services) is being organised. According to the standard 2 which is related with partnering with consumers. The main intention of this is to create health services that is responsive to their patient and carer inputs, requirement and priorities. This act provides health framework for all active partnership among consumers, clinicians and health sector organisations etc. There are certain consequences those are related with the patient and family under this incident are mentioned underneath: Patient: It is serious and undesired, such as death or unusual extension of hospital staff as seen in the case of Ruth Stoll. She died because of not having original blood transfusion. She need to suffers from underlying health condition or form a risk inherent in delivery health services. Family: This is basically related with how much care do you delivering to your relatives or patients. In the case of Ruth stall, her family was always remaining outside while the test and checks-ups. This is wrong from their side, as they need to always present with the patient which was not there. 4

It was based on certain conceptual framework that used to provide preliminary description of some decisions that are helpful for resolving any kind of matters. The use of technology can be more effective in testing any kind of samples that has been related with growth or stability of the patient. Take actions that consists a mandatory or voluntary notification to AHPRA, if a nurse knows or reasonably suspects that they or a colleague have a health situation of the patients (NMBA, 2018) Dept. responsibility: Lab testing department should work in this process.
