Introduction
One of the vulnerable group that requires attention from medical, nursing and
community support are elderly people that stay at home. The elderly population in the world
is increasing at a dramatical rate because of the improved in medical technologies and
continuous research over time. According to the World Population Ageing 2019, this global
phenomenon indicates there are 703 million people that are above the age of 65 years in
2019. It is estimated that it will double to 1.5 billion by the end of 2050 (Un.org, 2020). The
ageing population is observed to increase at a fast rate in the Eastern and South-Eastern Asia
as well as in Latin America and the Caribbean. In order to take care of elderly people residing
at home, various factors need to be considered and thus, it becomes challenging and unique in
nature. Due to age factors as well as other health issues, the immune system, mobility and
mental status of an old person is found to be unstable and deteriorating in nature (Sungur,
2017). The aim of this paper is to discuss the significance of taking care of older people at
home and having issues such as Down syndrome, exposure to alcohol and complications
from meningitis with respect to understanding the role of the nurse in the care process.
Moreover, nursing consideration needs to be taken into account while taking care of such
patient to offer a healthy and independent life.
Discussion of Rationale for caring the patient in the community
Mr Henry Jones is a 70 years old man who lives with his son, his wife and their
teenage daughter. He has Down syndrome from childhood, he was alcoholic from a very
early age of his life and continue it for a prolonged period and recently, he had developed
from meningitis a year ago. The complex health issues of Mr Jones had limited his
movement; it was found that due to his intellectual disability, and he requires support in
conducting Activities of daily living (ADLs). According to Johansson et al. (2017), the vital

contribution of the behavioural and mental disorder is important and it can be found that old
people having intellectual disabilities have a significant harmful impact on the affective,
cognitive and normal functioning of life as well as the quality of life. Thus, it necessitates the
need for taking care of the vulnerable group of people that support in enhancing the quality of
life as well as limiting the support of other people in leading a healthy life. This will, in turn,
helps in improving the overall health and well-being of the community and will help in
building a healthy population reducing the pressure of the healthcare sector on a national or
local context (Moffatt et al., 2017). Therefore, it can be stated that it is critical to address the
issues related to social determinants of health of the particular population that is old people
having several intellectual and physical disabilities thus, reducing the cost related to
healthcare in the country as well as global level.
Role of nurse and nursing considerations while caring
From the opinion of Engel et al. (2016), the geriatric care is stated to be complicated in nature
because it comprises of multifactorial impact on the individual, family member, healthcare
professionals and the society as a whole. Therefore, the role of the nurse in the care
management of old people such as Mr Jones is essential because of the development of care
programs. These are stated to enhance the coordination and integration of various care
services in the continuum of care, especially of weak old individuals. The use of various
models of care is taken into consideration that includes the cooperation of registered nurse
(RNs) and community workers for assisting in the care plan (Karlsson et al., 2020). The
nurses are to conduct different kinds of activities that include assessment, documentation,
allocation of resources, teamwork, data collection, referral, Occupation health and safety
(OH&S) practices, ethical challenges, family engagement and challenge in the care process.
The activities are stated to be critical while taking care of an old person with complicated
health and mental ability because an integrative approach is one of the healthcare frameworks
