Dear Mr. Oscar Murgor
I am writing to inform of the key findings from the assessment I made in Appalachian
community. It is a rural community in Ohio associated with high poverty levels and high health
needs. In my teaching plan, I realized a lot of factors affecting the health needs of the Ohio
people. These people are affected by cultural factors and geographical factors that make them
fail to access the free medical services available in the society. These people have thus suffered
various health implications that need immediate intervention. The health conditions they are
suffering include obesity, Conigetal heart diseases, diabetes, stress, hypertension an also stress.
This research was supported by data collected from the community where the general public was
involved in the research. Data was collected through interviews and focus groups and the
information gathered was analyzed through thematic analysis. The public health nurse role in this

plan is to offer the required nursing interventions to enable the reduction of the above named
conditions.
I have worked with your department earlier and I am aware of your immediate action when it
comes to the health issues in the community. We need the immediate help in Ohio community
and thus hoped that your department could grant us the best help we need. Thank you in advance
