Epidemiological Rational for Topic:
As per the study of Centers for Disease Control and Prevention (CDC), there are more than
100 million Americans who have diabetes or prediabetes. It was found that in 2015, 30.3
million people in the United States were suffering from diabetes (CDC, 2017). It contributed
to 9.4 per cent of the total population of the country. Along with these diabetes patients, there
are another 84.1 million Americans who have prediabetes. If they are not treated within the
period of next five years, then they are likely to suffer from type 2 diabetes. There are
approximately 1.25 million American children and adults who have type-1 diabetes.
Moreover, over 1.5 million new people are diagnosed with diabetes every year in the US. It is
the seventh leading cause of death in the country with 79,535 deaths in 2015. Nearly, 1 in
every 4 adults is living with diabetes which means 7.2 million adults are diagnosed with

diabetes (Diabetes, 2017). Furthermore, 11.6 per cent of the adults in American are suffering
from prediabetes.
Nursing Diagnosis:
The main reason for the increasing number of diabetes is imbalanced nutrition because body
takes more nutrition than required which lead to weight gain and it increases the threat of
diabetes.
Readiness for Learning:
The feelings of young adults will be evaluated for preventing type 2 diabetes along with
obesity. They will be asked appropriate questions regarding reducing risks factors, and they
will be encouraged to engage in conversations to improve their health.
Learning Theory to be Utilized:
Teach-back theory is an effective way to reduce the communication gap between the teacher
and the clients (Jager & Wynia, 2012). I will also use this theory to connect with my
audience. The teaching materials which I will use will be in simple English, and they will not
include complex medical terminologies. Clients will be questioned based on what they learn
from the brochure, and they will be asked to repeat the material. I will use eye contact, body
language and personal connection to understand clients and teach them about the threat of
diabetes.
Goal:
The focus area selected for this plan is “D-16 Increase prevention behaviors in persons at
high risk for diabetes with prediabetes”.
This goal aligns with my teaching which is focused on changing the behavior of young adults
in relation to increasing the level of their physical activities. This goal was focused on
encouraging 49.1 per cent people to increase their physical activity who are aged 18 and
above to reduce the risk of diabetes (Healthy People, n.d.). These individuals have fasting
glucose which is higher than or equal to 100 and less than 126mg/dl. People who are 18 years
or above age should reduce the amount of fat or calories from their diet to address the issue of
diabetes.

Initiatives?
The global initiatives of Alma Ata’s Health for All were focused on providing that major
health issues did not affect single countries and the health disparities between countries have
a negative impact on everyone; therefore, achieving health objectives of world’s population
should be the goal of all countries. The D-16 objective of HP2020 aligns with the global
initiatives since they both focus on increasing awareness and education between people
regarding how they can address and prevent the issue of diabetes (Nicholas & Breakey,
2015). Both these initiatives focus on improving the quality of life of everyone across the
globe.

Planned Evaluation of Objectives:
1) Changing lifestyle in order to reduce the threat of diabetes. The brochure will contain
details of exercise objectives regarding which exercises should be done for more than
20 minutes and six times a week.
2) Selecting right food in the busy life by making plans about future meals to reduce
obesity and the threat of type 2 diabetes.
3) Monitoring young adults in order to find symptoms of diabetes such as numbness of
fingers, blurry vision, pain in feet and feeling of thirst all the time.
4) Maintaining the blood sugar level within the safe zone to reduce the complications
caused due to type 2 diabetes. Improving hand hygiene, skin care to protect wounds
and regular blood sugar check.
Planned Evaluation of Goal:
I will continue to meet young adults in the church on alternate weeks for the next three
months to help them reduce the threat of type 2 diabetes. I will follow up from those attended
by program to learn whether they comply with the objectives or not.

An anonymous questionnaire will be given to those who attend the class regarding the
effectiveness of the program. Attendees can choose between 1-3 where 1 is agree, 2 is
neutral, and 3 is disagree. Many questions will be included in the questionnaire such as:
Do you understand the presentation?
Whether it meets your expectations?
Other questions will be included regarding the performance of the instructor and the quality
of the learning materials. Lastly, attendees will be asked to give their feedback regarding
what was the issue with the presentation and how they would have improved the overall
presentation?
Barriers:
The lack of willingness and inability of people to comply with the recommendations are
major barriers which may arise during teaching since people did not prefer to change their
daily habits. People might be in denial regarding their current habits, and it will create
barriers to reach them and influence them to change their habits.
Communication:
I will start the presentation by introducing myself and offering snacks to the audience. I will
later give them a brief about what is type 2 diabetes along with statistics regarding the same. I
will show diagrams about how type 2 diabetes leads affect people and its negative
implications. I will then ask the audience members regarding which snack did they choose
and teach them about the potential health benefits and negatives of healthy and unhealthy
food. I will engage them in the process by asking particular members regarding their
experience with this disease and whether they are right or wrong. I will also bring a young
adult on the stage who is suffering from prediabetes to tell the audience about what is his
daily life is like and how does it affect his future goals. Lastly, I will demonstrate how
audience members can keep track of their health to maintain a healthy blood sugar level and
encourage them to ask any questions regarding any queries which they have regarding the
presentation.
