Multidisciplinary teams are effective which can help to get better delivery of healthcare
services which are associated with meeting individual complex care needs. This include different
healthcare professional who are having expertise in their field. Here, multidisciplinary team
should work together to ensure about facilitating different care needs which include proper
management of declining health due to stage 4 (metastatic) bowel cancer. Multidisciplinary team
should ensure about cooperation between different specialized professional to provide related
treatment and care for the stage 4 bowel cancer. They should work collaboratively which can
provide better health improvement in the Mr. Joggers health (Patel et. al., (2020)). They have the
goal for improving treatment efficiency with patient care. There is also need to take care about
the pain which is getting worsening due to which Mr. Jagger wife has taken him to the hospital.
Here, multidisciplinary team should involve in delivery of better and effective healthcare
treatment to Mr. Jagger and help to provide best possible treatment. Nursing staff should also get
involve in fulfil the needs and support which should be provided to Mr. Jagger. There is also
need to involve Mrs. Jagger that can be helpful in providing instruction which should be taken
care of effective response towards dealing with better care and support. Nursing staff need to
provide knowledge and learning about the proper care management of Mr. Jagger declining
health. This can be helpful for Mrs. Jagger to provide proper care at home which can also be
effective to ensure about health improvement. Through learning about type of care, delivery of
medication and other management process which should be carried by Mrs. Jagger can be
helpful to provide better and effective response toward the managing health. Palliative care
should be delivered to Mr. Jagger that can allow to improve health and provide best treatment
and care that can help to reduce the worsening of health or try to manage those to minimise the
negative impact (Pandeva et. al., (2019)).
There is need to include different factor like advanced care planning, pain management,
nursing assistance through holistic approach and others. Here, multidisciplinary team includes
healthcare professional like oncologist, pathologist along with radiologist and nurses. They all
are involved to ensure for health improvement. Nursing staff and healthcare professional should
be involved in planning and arranging Mr. Jagger’s home return. These are effective to take care
of individual health progression. Here, they should perform advanced care planning together to
effectively reduce the pain and healthcare management that are associated with palliative care.
