PATIENT PROFILE
Mrs. Emma is 68 years old who is a single mother of three grown child. She maintain her
job along with care for her child. As she is self employed person so that she can manage one of
his child who is facing disability (Arakelian and et. al, 2017). She is facing early symptoms of
hip pain which restrict her do take care care of his daughter and to do her job (Patient Experience
Case Studies, 2020).
PREVIOUS MEDICAL HISTORY
The patient was taking medication from past several years as she was experiencing hip
pain which leads to unable from walking. The physician also provides her dose of her hip pain as
she was facing problem in walk. After it, she complained of having more pain with in her body
to physician. This result in increase of her pain occur as increase in medication of her dose
including prescribed medication for arthritis. It also include deterioration in working capacity of
Emma (Pettersson and et. al, 2018). She consulted with her physician after being not recovery
from medication and who suggest her to take consult from ODP. The ODP told her that she has
to come under some test so that her actual condition can be determined. After being tested with
CT scan which is six month later because the ODP was only able to see her in two weeks after
being tested. As the CT scan test describe the condition that her hip joint are importantly
deteriorated.
PRESENTING COMPLAINT
The Emma is facing problems in getting surgery as her condition is deteriorated. She was
put on a waiting list and told her that she would get her surgery as soon as possible. The months
has passes and she is not aware about any scheduled of her surgery. She was proposed to consult
from client representative but she also attain same response from them that she is on a list
(Sawatzky and et. al, 2017). This lead to a proper 24 months of period in her surgery. This
develop concern about over all health of Emma as she broken down in crying.
DURING PRE-OPERATIVE APPOINTMENT
The patient is following up from weeks and her appointment is scheduled from that day.
She was scheduled for pre-operative appointment and during this appointment, she communicate
with lots of health professionals who provide consultation and conduct different types of test,

assessment to her. With the progress with in her treatment, she feels overwhelmed about it that
she receive information about her condition and surgery. The different pre-operative test are
conduct before the day of her surgery, on her to ensure that she is still is a candidate for surgery.
After it, the hospital contacted her that her surgery is rescheduled due to availability of low bed
with in hospital. As her bed was given to an emergency patient for overnight. This leads to
several days of waiting and after with long patience she was informed that her surgery is with in
week. She also had fear of cancellation of her surgery again (Pettersson and et. al, 2017).
MANAGEMENT BEFORE, DURING AND AFTER SURGICAL
During her surgery day, she was prepared for surgery by performing various test by
operation department practitioner so that her surgery can occur properly. The surgery of Emma
was well managed by ODP including her pain throughout her stay in a hospital as her surgery
runs smoothly. Their are some effects occur after her surgery as she feel immediate urinate. This
was manage by perioperative nurse who provide appropriate care to people who are undergoing
surgery. The nurses provide immediate help to Emma so that they can assist her. At the time
when she feels urinate intensely then she call nurses but she was not immediately respond which
develops curiosity in her (Leo, Jangland and Arakelian, 2017). This leads to pressing the button
again and again. After it, she urinated all night with in bed as she was afraid of being impose on
nurse staffs. After her second day of surgery, discharge planner come to discuss about his needs
that if she wants extra care and support in home. They also discuss the need of physiotherapist
which is necessary for a patients after being surgery.
DISCUSSION
There are various difficulties that are faced by Emma from her perioperative treatments.
Mrs. Emma has different difficulties when she was consulting for her condition to
physiotherapist. As while consulting for her pain, her physician are not much concerned about
her pain and they give medication to her pain. They are fail to recognise the intensity of pain
with in her body as this was not effective care of plan as this does not provide navigation to
system of organisation. With this, it was not proper plan that is designed for Emma care of plan.
The patient have to wait for several months from which develops impact on her life. As the
patient have various previous complaints regarding his health condition. As she have to wait for
longer time to consult from her physician. The lack of information is provided with in procedure

of his treatments which result in impact on her life. As she is not provided proper information
schedule of her electric surgery and the date of it has pushed back. She does not receive
appropriate information until she had to call client representatives (Jun and Oh, 2017). Mrs.
Emma explained to them that she does not receive proper information about her surgery and has
been delaying for several times which built as she has no idea about her schedule of surgery. In
the case of Mrs. Emma, she was accompanied by health care assistant which has no specific
experience towards particular needs of patients. As she was asked several times from them to
about her schedule of surgery (Keller and et. al., 2017). They also does not informed the date of
surgery that has scheduled. This leads to deteriorate condition of patient as she is not receiving
appropriate care and support before her surgery. The patient must attain proper care options so
that her condition can manage while wait. As it can develop various other problem with in the
body of patient which lead to impact on surgery. The development of other disease can cause
severity in undergoing surgery. But with in case of Emma, she does not have other impact on
body which lead to severe impact on surgery. When she arrived in operation theatre then she was
accompanied by health practitioner of operation theatre who does not have proper knowledge
about patient. She asks various questions about her anesthetic but the practice fail to deliver
information about it. Due too late in a surgery develops various impacts on her mental state from
which she repeatedly asking about her surgical process. In the operation theatre, when anesthesia
is providing to her she repeatedly asking question about her process of surgery as she highly
concerned about it. The operation practitioners are also unable to analyze the situation of Emma.
Due to improper care of plan, she does not move freely and she was lived with her pain for two
years. In her communication with her physician, she wan not fully engaged with it which
develops negative impact on her health (Pereira, Figueiredo-Braga and Carvalho, 2016).
Including this, she was also not determine proper care and action plan from which she can
minimize her pain. The complete diagnosis of her treatments is not appropriately determined
(Inge and et. al, 2017). Their are time overflow is occur with in hospitals which develop bed-
spaced for medical patient with in surgical wards. This cause delay in surgery of patient which
result in cancellation of surgery. The perioperative care was provided to patients. As in this, she
communicate with various health professionals which develops sanctification and hope among
patient. The each patient have their own needs and requirement which is necessary to be consider
by perioperative nurses. When the patient is challenged from her socially then this result in stress

and frightening environment for her as she has a responsibility for her daughter (Graham-
Wisener and et. al., 2019). The patient feels down when she was delaying in her proper care plan
which develops various other impact on her surgery. As patient is facing many challenges
because she is unable to walk properly which result in developing more difficulties in her delay
of surgery.
Silva et. al, (2016) considers that the staff is rely on ward nurses for providing
information to their patient. They need to make confidentiality while providing services and
information to their services. They need to cover personal information of their patient other than
surgery and observation. The team members sometime loss the fact that each patient have
personally needs of care and services. In case of Emma, she wants to recover as soon possibly
because she had to manage her daughter whom she has disability. The responsibility of her
daughter are provided to her son but this develops conflicts between them. So, Emma wants her
surgical treatment as soon as possible but delay in her treatment cause various impact on her
situation as she start crying due to delay in her treatment. After being delay, she rely on help
from advocate so that her case of surgery can be move on (Thomsen, Soelver and Hølge‐
Hazelton, 2017). The Emma consulted various times to her physician and show concern about
her condition of being more deteriorate due to intake of medication. As she was suffering from
her disease for two years by taking medication but after arise of conflict between her family, she
urged her physician to consult her a ODP so that she can minimize or remove her pain with
consulting with her. The plan of care contribute to the experience of actual patient. As in the
starting phase of her disease she face lots of difficulties in her treatments as physician does not
show high concern about his treatments. She was continuously asking about her surgery
scheduled to professional but it is delay from various time. The Emma spend her important
amount of time with in pre-admission clinic so that she can meet with individual professional
(Lauti and et. al, 2016). She has to wait more longer with in her meeting but end of the day, she
is provided to all information that she wants to receive (Zhou, 2018).
Their are various issues occur with in plan of care of patient such as lack of
communication is available on the day of procedure of surgery from which her surgery has been
cancelled. The plan of care is not proper planned which indicates that the coordination between
service providers for simple things such as scheduling and providing information (Koerner and
et. al, 2017). The patient experience friendly staff while she is having her surgery as her

procedure of surgery also occur smoothly. The pain of her hip was managed properly after the
surgery. Their are some individuals who leave a long lasting mark on the experience of
treatments. Their are various effect occur after her surgery such as more intense urinate. At that
time, no nurse is provided to her which develops problem among her in urinate. Their are
number of lack of leadership occur with in clinical area which develops dissatisfaction among
patient and decrease the quality of services the provides a negative impact on health of patients.
After being feel intense urinate, the patient called nurses for her help but they refused to do so.
Mrs. Emma asked nurses about her needs of urinate to them but they greeted her and gave
answer that “We can’t be in two places at once, dear (Marion and et. al., 2017). This develops
afraid among Emma as she does not want to impose on the nursing staff (Baumbusch and et. al,
2018). The family physician of Emma must communicate with her by asking different question
about her pain and mobility. These are various difficulties that are face by Emma in her proposed
plan of care. But after the surgery, she feels supportive with in her surgery as at time of
discharge. The discharge planner discuss with her about his needs including support of
equipments that is needed for her after being discharge at home. The proper plan also discussed
about physiotherapist as per her physician's orders. The services of physiotherapist is arrange at
her home which feels securely after being operated. The proposed plan that has occur during her
surgery has created a positive impact on her health as she feels more recovery towards at home.
As proper care plan is proposed arrange by providing physiotherapy services at home.
Haakseth (2019) discuss that their is need of providing leadership in clinical areas as
with this, satisfaction among patient for their services can be enhance. This was an undignified
experience for Emma although sh does not mean to had experience such circumstances. The
hospital provides maps to her so that she can guide about her surgical procedures. The
experience of Emma with in hospital is smooth and it is carried out from the transition of follow-
up care to home. The plan that has proposed after being admission her in hospital was very well
managed by following up after discharge. The consistency also occur with in staff from which
they conduct themselves (Clarke, Nightingale and Cunliffe, 2018).
Mrs. Emma found difficulties initially in corporation with physician and team members
as she does not receive proper concern in her initial stage of pain. The physician does not ask
question about her condition which is later on consider by ODP after being tested of her hip pain.

the patient care plan are planned to improve services and care of all patients who are
hospitalized. They have proper aim of sharing information and improving their communication .
This develops involvement of everyone for caring patient by working with the same goal. The
care plan includes provide confidential services, care and support to patient with in their house
and hospitals. The patient care plan are not prior to share with all staff who are providing care
and support to home. In the case of Emma, she experience various things with in her surgery
such as the information that are provided to her must be confidential and not disclose to other
people. This significantly important as it provides needs of patients, so that proper
accommodation can be deliver to patients. As this helps in developing more satisfaction about
services and care among patient from their plan of care can be successful. The safety provides
baseline of care as safe operation is the need of patient. Mrs. Emma was facing various problem
initially when she was consulting her physician. As the physician does not provide proper
concern to her for her pain. But after being admission in hospitals then she receive proper care
and plan during her surgery and after surgery. The delay cause in her surgery which develops
deterioration in her health and pain. But her operation run smoothly and became successful.
However, there are number of personal needs are necessary according to their situation which are
not consider properly. This lead to challenging experience for Emma with in her surgery which
develops various impact on her. She also faces various other experience that challenge his mental
health (Denard and et. al., 2018). As she wants her recovery done as soon as possible so that she
can mange her daughter properly which has to manage by her sons. This develops various
conflict between them which she wants to remove by her surgery so that she can handle by her
own. So the staff members need to aware about her needs so that they can deliver care according
to them. If they were aware about it then they can provide earlier treatment and surgery to her so
that she does not feel abandoned from them. The staff members should consider Emma when she
was in the list of surgery. They must aware about her needs so that they can provide proper care
to her. With proper care planning, they can provide proper information about services about her
surgery so that she can feel continuity about procedures of hospitals. This develop satisfaction
among patient from by awaking about his needs and requirement of being operated. The
physician must communicate with her appropriately so that her concern of being treated properly
can be achieved with in given period. This enhance aim of proper care of plan from which they

can fully successful their aim. The staff member of hospital must improve their communication
about sharing information. This has them in providing information to their patient related to their
surgery taken seriously deliver clinical needs of patients. Her surgery was successful by staff
member who care her doing operation.
REFLECTION
This case provides me to observe important aspects of proper plan of care that is needed
to provides care before, during and after surgery. As I see that each people have different needs
and circumstances which is necessary to be develop while carrying out proper plan to patient.
There are more things occur which providing care to patients rather than completing task. I have
seen various working styles of professional with in their busy list from which it is not easy to
fulfil needs and requirements of each patients needs. It is hard to manage their needs by giving
proper and appropriate care to them with in proper time. While providing and consulting patient,
it is necessary to listen them carefully so that treatments can provide with them in a given time
period. It has faced with situation from which I can stop situation and take a minute to listen and
talk with patient. So that they can share their problems with me which assist in providing
treatments in proper duration. It is hard for student to stop action of others but there is a time and
place come where I can take step against action of others. In future, I will ask to operation
department practitioner about their lack of present which delays in surgery of patient. I
appreciate myself to learn about reason behind action as I am new to environment. As if I don't
ask then I don't learn. I have learned that it is important to ask question so that relevant
information can be developed. As the information does not relate with availability of it. The
asking different question can help in determining communication among patients that is need for
them to know about their situation. The team leaders must share information further so that it can
effective to me. Moreover, I learn various things by analysing situation about carry out needs and
requirements of patients when they come up for their services with pain. This also assist me to
deliver needs and care from which I can see further information regarding their treatments. This
would let me to proposed proper care plan so that I can provide care before, during and after
surgery of patient. This guide me to take and provide proper information between patient and
them so that their confidentiality can be maintained. It also provide information about fear that
patients have when they come in theatre for their services. This case provides me information

about importance of sharing communication and making coordination with patient. As it is
necessary to ask question and deliver answer within operation theatre so that needs and
requirements of patient can be address properly. This also provides reflection of operation
department practitioner from which I can learn about it less effectiveness of care in ODP. This
provides me guidance to take proper steps in perioperative phase by proposing proper plan of
care to them.
