As per the medical record standards, chartsand documents of medical documentation enable accessibility and confidentiality over patient'sinformation which require the following details which are described as: Individual record of patientRecord of every visit and prescriptionEnabling security to electronic medical recordsFurther, documentation and charts of medical record involves every bit detail aboutservice users which is a list of health issue, current illness and medication conditions. Apart fromthis health practitioners are focused on documenting about prescribed medications, allergies,smoking habits, impact of drugs, past history of alcohol intake, history of pertinent, reports ofdiagnosis, plans for further treatment, record of immunization and screening of risk (MedicalRecords and Documentation Standards, 2018).
