PART 2 Potential and actual medication related to disease of Mr MR From the Mr. MR case study it has been analysed that is feeling fatigue and several other problems related to this also identified. Some problems from which he is suffering include: weight gain, constipation, feeling cold, loss concentration and fatigue. After that he was diagnosed for hypertension, hyperlipidemia,hypothyroidism, heartburn and epilepsy. From hypertension diagnosis it was identified that his Bp was normal, tests of lipids also shows that it were fine as per to his general profession. But in the context of heartburn which he diagnosed it is identified that he often face this problem especially after having meals and that is why he reduced consumption of food (Hobbacher, 2016). From his medical history it is also identifiedthat he was taking some medicineswhich includesRenitec,Tegretol,Cavstat, Gaviscon and Cool liquid.After identifying and evaluating all problems, diseases and previous medical history, now some potential and actual medications can be suggested and prescribed to the patient related to his all problems such as: Fatigue: There are several medications are available for fatigue but as per the history of Mr MR, some best medications which can suggested include: Bupropion,sertraline and modafinil off label are some medications. Ferro grad C can also be suggested which patient was taking already as it consists of vitamin and Iron and it is also identified that patients was not facing problem in having this medicine (Yadollahi and Shamsaei, 2017). Heart burn: It is one of the main problems from which patient is suffering from and needs to solve in a timely manner. It is identified that it increases after having meal or food. Antacids is one of the potential medication which can help this patient from heart burn or neutralize stomach acid which can relief pain and burn. In addition, lansoprazol can also be suggested to the patient which is proton pump inhibitors. It can also help this patient in reducing stomach acid and making him relief from heart burn (Ekinci and et.al., 2019).Gaviscon Cool liquid can also be suggested as patient needs to take this after having meal. 1
Constipation:It is one of the main problems which patient stated and it is related to heart burn as well. Some natural remedies can be suggested to the patient like drinking a lot of water, focusing on exercising, eating fibre enriched food and drinking coffee. polyethylene glycol is one of the best and effective laxative solution which can help Mr MR in solving problem of constipation by increasing amount of water in the intestinal tract. Weight gain:There are some drugs are available which can help patient in reducing weight as his weight is 102 Kg which can create several other problems. So, for reducing weight, it can be suggested that patient should focus on exercising, having food with lower fat. Orlistat and lorcaserin medicines can also help in reducing weight. Hypertension:It is stated that BP is normal but patient was diagnosed this before 12 years and from all other problems it is important to suggest some medications related to hypertension. For thisthiazidediureticcanbesuggestedbecausethereisnopresentorcurrentsignsof hypertension. Hyperlipidemia: this problem was diagnosed 6 years ago. According to the GP hislast tests for lipids were fine. For this, he was taking Cavstat 10mg (rosuvastatin) medicine but used to occasionally miss it. This occasionally missing of medicine can result in in increasing cholesterol which can further result in weight gain (Verma, 2016). In addition to this he was overweight and used to drink on weekends and have takeout means which can be one of the main reason because of which his cholesterol can be increased and increase chances of heartburn or weight gain or constipation. It is recommended to either change his medicine or review dose taken by him and educate him of not missing any of the dose of this medicine. Working uponHyperlipidemia condition can reduce changes of constipation, weight gain, heartburn, high cholesterol fatigue. PART 3 A letter outlining findings and recommendations to the patient’s GP Dear Doctor GP, Mr MR who is a 62 years Asian male who is one of your patients presented to the pharmacy today. He is one of my regular customers. He presented his prescription of thyroxin medication which is majorly used for hypothyroidism. He was feeling some fatigue and because of which he asked for somevitamin’sproducts. From these findings I had analysed my findings and identified that as per his current age and high his weight is quite high and due to his increasing weight it can become dangerous for him. I had identified that his consumption of Ferro-grad C (iron and vitamin C) dose should be reduced. I think daily one dose of Ferro-grad C consumed is quite high because of which he is facing issues like heartburn and constipation. It is because constipation and heartburn these are two most common side effects of overdose of iron products. Other than this, from my assessment I have also identified that he quite frequently forgets to take his Cavstat 10mg (rosuvastatin) which is used for reduction of cholesterol. It is another factor because of which he might be suffering from such issues. This is majorly because he said every weekend, he has takeaway meals and have 1-2 beers in the nights every weekends. Occasionally missing dose of Cavstat 10mg with takeaway means every weekend with 1-2 beers in the nights on weekends can result in increasing cholesterol. Due to which he is suffering from weight gain, constipation and heart 3
burn. Missing some of the doses of Cavstat with every weekend takeaway means and beer can result in cholesterol is one of the main reasons of his increasing weight gain and loosing concentration. It is my assessment that his fatigue is also because of his increasing weight. My recommendation for Mr MR’s current condition and medicine is that I think her medicinesarerequiredtobereviewedagainwithappropriatenumberofdoses.His Hyperlipidemia with lipids test is required to be done again as I think that her cholesterol is increasing which can be result her weight is increasing, is having constipation and suffer from heartburn.It is my recommendation that testing these conditions and reviewing her medicines will help in identifying main reason because of which his weight is increasing. Cause of weight gain and working on it will help you in reducing constipation, fatigue situation. Which will also help in eliminating requirement of taking Ferro-grad C (iron & vitamin C) medicine. Reviewing Hyperlipidemia and its medicines can also help in reducing issues faced by Mr. MR such as heart burn. This will also help in reducing her number of medicines that are taken by him at least his iron products and heart burn medicine use can be reduced. In order to increase results of effective treatment I think it is important to continuously monitor his condition and a regular follow up should be done in which his current condition, medicines he mis to take, additional medicines without prescription that he takes should be reviewed by time to time. Other than this here are few recommendations that can be used to bring improvement within condition and reduce issues faced by them: first is to educate Mr. MR all the medicines that have been prescribed to him and what can happen it he misses any dose or overdose any medicine. He should also be educated about which medicine should be taken by him empty stomach and which should not be taken empty stomach and what can happen if he does not follow this guideline. Please review all the combination of medicines prescribed and taken by himself as it might be useful to understand reason because of which such issues are faced by him. There is a recommendation that please review his doses that are taken by him and when it is taken. These are few suggestions from my side that can be focused upon and I will be happy to discuss his condition if it is required in future. Yours Sincerely, 4
