Community assessment
The Central London Community Healthcare NHS Trust situated in the England, and they work
on the health care for the betterment of the people (Christie, Gray, Dumville, and Cullum,
2018).
Community profile
Central London community healthcare provide a service to approx more than 2 million people of
the country (Panagioti, and et.al 2018). Every the community train the people to avoid unhealthy
things and stay healthy. Organization vision is to deliver the safety measure and care as much as
they can provide to the people and mission is to work together in dynamic environment and give
the children a better start by giving them a healthy environment.
Monitoring and surveillance
Monitoring refer to the systematic approach used by the community to settle down the people in
order to sustain them and make them healthy in the country. This involves the observation and
understanding of the people and their behaviour.
Surveillance includes the targeted monitoring by analysing the policing factor and security of the
people. Surveillance mainly focus on the individual and their information by which this helps the
community to identify people who need help.

Public health England includes the life expectancy of male, female and disease person of the
England by which it helps the community to identify the people. They want to help of different
category (Lachapelle, 2020).

This include the four stages which determine the demographic transitions.
First stage include the high population growth which is better stage. In this stage the birth rate is
high and death rate is as well.
Second stage is known as population explosion by which the death rate is decreases and birth
rate is constant.
Third stage is included the birth rate which identify the reduces and the death rate will remain
constant (Rosas, and et.al 2021).
Forth stage include that the rate of both birth and death are low or remain constant or have the
same level.
Need:- the need of this theory to the community is necessary because it indicates the essential
elements of the country which community need to help the people.
Asset:- the main asset of this theory is improvement in the mortality rate than fertility rate. This
process increase in the short- lived increase in the family and their size.
Demographic change:- this indicates the Change which help the community ion order to
development of the people and the country by the community and diversification in different

stages. In demographic change typology will be helpful this indicates the process and the
opportunity to make difference in the countries and their birth rates.
Epidemiological transition:- epidemiological transitions a process by which this identifies the
mortality and those who have disease can be transfer by the high mortality rate among the new
born and the kids and also in the female rate growth. This lead to demographic change by which
it consists reduce in child birth and because of this it includes reduce in fertility rate. In the year
of 2020-2021 there is so much decline in the mortality which causes the health measure and the
development of the country (Mercer, 2018).
Nutrition transition:- the nutrition transition is used by the researcher for using the
demographic theory of population used in shifting the stage 3 of the demographic theory to the
stage of demographic theory. This includes the diet and physical; activity by which people can
cure their diseases. Nutrition transition include so many things such diets that they include is
western diet for such people who like this because this is used for it to change the traditional diet
of the people into the western (Sievert, Lawrence, Naika, and Baker, 2019).
