Community assessment
The Central London Community Healthcare NHS Trust situated in the England, and they work
on the health care for the betterment of the people (Christie, Gray, Dumville, and Cullum,
2018).
Community profile
Central London community healthcare provide a service to approx more than 2 million people of
the country (Panagioti, and et.al 2018). Every the community train the people to avoid unhealthy
things and stay healthy. Organization vision is to deliver the safety measure and care as much as
they can provide to the people and mission is to work together in dynamic environment and give
the children a better start by giving them a healthy environment.
Monitoring and surveillance
Monitoring refer to the systematic approach used by the community to settle down the people in
order to sustain them and make them healthy in the country. This involves the observation and
understanding of the people and their behaviour.
Surveillance includes the targeted monitoring by analysing the policing factor and security of the
people. Surveillance mainly focus on the individual and their information by which this helps the
community to identify people who need help.
