1. It is identified that George suffering from hypertension diabetes and also a regular smoker.
There are a number of complexities and risk that are associated with the condition of George
that has to be taken care for start the primary is associated with his diseases are related to
impact on various body parts such as eyes kidney nerves heart and blood vessels. The primary
impact on the eyes can lead to swelling within eyes and can make permanent blind for stop
along with the risk associated with this disease to the kidney problems where reviews of and
improper control of blood sugar and high blood pressure will lead to kidney failure. This can also
lead to several nerve problems where people who have diabetes for long period can develop the
type of nerve damage diabetic neuropathy. The diabetic neuropathy can impact on the nervous
system of the patient and different body parts. In this case there are several practices that are
necessary to be performed by me as a nurse. These are looking the signs and identifying the
problem within the patient. Along with I will also use to restore the balance of fluid electrolytes
and acid-base balance. I will also use to correct device the abnormal metabolic function and help
in managing the underlying cause of diabetes and disease process. I will also try to prevent the
diabetic complexion where educate the patient about the diabetes and hypertension effect on
their body and provide them self-care education.
2. There are a number of potential impacts of patient condition in situation under this disease
where it is necessary for the nurses to use appropriate strategies and action. If the patients
suffer from any kind of the disease mentioned above or there is increment within diabetes or
hypertension then a particular process will be followed by the nurse. The process will include the
guidelines that have to be followed within the high potential. The most common problem with
this kind of patients is related to heart attack. In order to prevent the heart attack and provide
effective solution to it that practices that has to be used by the nurse include providing CPR.
Along with appropriate medication and calling the emergency services. It is a most essential
strategy where a nurse has to be performed to achieve effective results within the patient
health. If the condition of patient get version then the patient will be admitted to emergency
volt of hospital where a proper treatment will be provided. In between the shifting from home
to patient what there will be use of number of practices by the nurses for stock these are related
to providing the oxygen marks and basic injections that are required to provide consciousness to
the patient until they reach hospital ICU.

Select two case studies from those supplied below. Follow the instructions provided and address
all the following three section (A, B, C and D) in your assignment. You may use this workbook
or complete in a word document. Either way, your document or workbook must be submitted
through as a PDF.
Section A (1000 words: 500 per case study)
Consider the nursing management of two patients (out of the four case studies provided in
Section A at the end of this workbook) and address each under headings provided below each
case study. You will be required to provide references to support your nursing thoughts/actions.
Case study A: Community care
You are working in a community health clinic in Dubbo. George, 47, has presented with
shortness of breath and high blood sugar. He has a history of hypertension, diabetes and
smoking. He is overweight and has a sedentary lifestyle.
George has a large family who reside with him in social housing. This includes grandchildren
and cousins. George does not have high literacy levels, is 47 years of age and has a great sense
of humour. He is a gentle caring man and proud of his ATSI heritage and his family.
1. Identify primary concerns re: the risks/complications that could arise from the patient’s
associated condition and your nursing thoughts/actions.
2. Discuss the potential impact of the patient’s condition/situation on the patient and
significant others and your nursing thoughts/actions.

1. There is lots of risk that are associated with the problem of the patient as she may be
suffering from tetanus or other disease. Under the proper evaluation is necessary in
order to this we will require with basic diagnosis of her body. After the diagnosis it will
be clear that what the actual problem she is facing. Under this it is identified that some
of the major things that come in the mind after hearing the case study of her is tetanus,
ammonia, and viral fever. Under the situation the primary conduct of a nurse is to
consult with experienced doctor where appropriate medicines will be provided for
saving the life of individual suffering from the disease. They will also use of blood
samples that must be taken from the patient and sent to the laboratory for checking
different diseases that can be e reason for this cold and difficulties faced by the patient.
There are several complications that can be seen within the patients as major t of these
complications are broken bones broken long artillery and that of the patient. Here it is
necessary for the nursing staff to provide appropriate care where social support and
mental support is also necessary for the patient and order to make them alive until the
doctor visits them.
2. Clostridium tetanus is the product of toxins produced in soil, dust and animal faeces by
spores of bacteria. If the spores are wounded deeply, they become bacteria, which may
trigger a potent toxin, tetanospasmine. The poison affects your muscles' nerves (motor
neurons). The toxin can induce muscular rigidity and spasms — the main symptoms and

Case study B: Emergency Department
Rupa Deepak, 37, presents to your emergency department with small scratch on her left wrist
from a rusty nail. The wound appeared inflamed and Rupa tells you that she and grandfather-in-
law were sorting through an old crate of junk that was to be sold at a vintage fair when she
injured her wrist on the nail. Thinking nothing of it she put a band aid on the injury and
continued sorting the crate. In the days that followed, Rupa’s husband reported that she has
developed a cold, it got rapidly worse, and when he returned home late after working he found
her sick in bed and as "cold as ice". At first, he thought she had "pneumonia or something".
On examination, you note that Rupa was also experiencing shivering and muscle pain, her skin
was a bit mottled, her speech had become a bit slurred and she did appear confused at times.

reduce medical error and contribute to the better outcomes of patients. This section discusses how

intent, communication styles, obstacles to productive communications, communication models and
techniques for communication improvement and clinical engagement recommendations for
progress.
Section B (1050 words)
After completing your two case studies from Section A above, consider the NMBA Professional
Standards (as provided below), and how you could demonstrate these through the nursing
management of your chosen two patients as discussed in Section A of this assignment.
