1 PROGRAM DESIGN PLAN AND EVALUATION This is a community health awareness program on the Kentucky community where almost no health awareness program was not been used. The stakeholders of this program would be the community people, pike country residents and the Kentucky health care team that are the nurses, physicians and health care personnel (Thornicroft, Deb & Henderson, 2016). The resources and the input of the program would be dependent on the pharmacists, nurses, physicians and the social workers along with the Government organizations of health and the Government unit leaders. The program would be done on the basis of small workshops for the community people each week that comprises of 12 to 16 participants. The workshops will be on the awareness about the medication importance, different daily exercises will be demonstrated and the dietary role in the daily routine also needed to be demonstrated in order to let the community know about the importance of healthy living and prevention of chronic diseases (Liddy et al., 2015). The awareness program will be evaluated on the basis of the outcomes of the program that is the individual skills and knowledge will be gained, family knowledge will also be gained by this program and most importantly the self management skills can be gained by the implementation of this program. On the basis of this assessment the outcomes can be evaluated as the mortality rates among the older community people will decrease and also negative impact of the lack of knowledge on health will be reduced as the knowledge about the healthy management increased. Implementation of this program needs manpower at least up to 50 people and the equipments and other resources would need a assessment of financial budget and the expected budget will be approximately $58,000. Finally the evaluation of the program will be held on the basis of the expected outcomes and on the basis of the evaluation the failure or the success of the program will be marked.
