Source of referral- Local council’s emergency relief worker
Reason for the referral- Tom Banks had been visiting the Local council in a row, to ask for food
voucher and financial assistance with the electricity and gas. Tom has left his job two months
ago to help his wife looking after their baby suffering from Down’s syndrome. The local
emergency relief worker of the council suspects that there had been some family issues that is
affecting their financial status as well as their physical and mental wellbeing.

information was obtained, whether others were present at the time)
Local council’s emergency relief worker- she had noticed that Tom had been visiting council for
second week in a row asking for food vouchers and money for paying the gas and the electricity
bills. The social workers has informed that Tom lives with his wife and three children. Jenny has
been finding it to be too difficult to look after baby Christie, who has Down syndrome. The
family had always struggled to make their ends meet. Any of the family members might have
been involved in gambling. To is ashamed of Christie’s disability.
Primary school principal- Tom has not been too friendly, but is definitely not a bad father. He
makes Sarah to stay back at home that hampers her studies. Terry is does not like to see his
parents fighting and often cries about his Mum. Terry stays in his world most of the time. He has
not made friends and has not even learnt the basics of reading and writing.
Sarah is nice kid, but often drifts off in class, and his grades had often gone down as she often
misses class.
A women in the Supermarket- The women told that tom fights with Jenny as the women have
heard arguments and one day Jenny had a big black eye one day, which might indicate towards
family violence.

Presentation During Assessment (Orientation to person/place/time, mood, affect (eye contact,
facial expressions, tone of voice, rate of speech, and gestures and whether affect is broad,
restricted, blunted, flat, labile or inappropriate, engagement)
The orientation of the person was normal but he was rude to the social worker as he failed to
show welcoming gesture to the social worker. During the initial conversation, he also yelled at
the social worker. His overall gesture was restricted as he repeatedly kept on mentioning that he
do not need any advice or help to support his family as he considers himself to be well-aware of
that. All he need is financial support. His behaviour at times seem inappropriate.
