Case
Mr James Cox is a 79 year old man who suffered a stroke and recently damaged the
posterior part of his brain. Individual has fallen down in kitchen and his frontal bone has got
fractured. After his accident he was unable to pay attention on anything and was in confusing
state. Patient has been admitted to rehabilitation centre for treatment. Medical professorial was
facing trouble in providing him treatment because due to accidents cognitive ability of the person
got affected. That is why individual is unable to concentrate on rehabilitation programs.
Diagnoses assessment and determining cognitive dysfunction
In the case of James Cox, patient is suffering from stroke (Solms, 2014). In such
condition blood supply in brain get affected due to which patient actual level of consciousness
get affected, that creates confusion situation for the patient. That happens in the case of James
cox. Due to accidents his forehead bone has got fractured but stroke is the main reason of his bad
health. In this disease individual face onset of weakness and half part of body fail to perform
well. Same happens in the case of James as well that is why he fallen down in kitchen. As in
rehabilitation centre he was unable to concentrate because in stroke patients vision can get lost
partially thus, individual fail to concentrate on one thing properly (Parkin and Leng, 2014). Thus,
it can be said that stroke is the main reason of cognitive dysfunction in Mr. James Cox.
Furthermore, age is another major reason for the condition of patient. As individual is 79 year
old in this age brain of individual fail to perform well and if small accidents occur then it impacts
badly on brain and entire body. While planning for the diagnoses of the patients , medical
professionals have to consider necessary these factors. These elements are such as age, mental
status, body responding capacity etc. By this way doctors can be able to provide correct
treatment to the person so that individual can get recover from their medical illness.
MEASURE ATTENTION AND GENERAL COGNITIVE ABILITES
As due to stroke and old age brain of Mr. James is unable to perform properly. Individual
is now faced difficulty in understanding speech and concentrate on the program. Thus, it impacts
negatively on ability to participate in rehabilitation programs. People over 55 year face high risk

of stroke as compare to younger people. In this condition individual's muscle movement fail to
react. Individual is facing difficulty in talking. Memory loss and thinking difficulties are the
major issues due to which James is unable to participate in rehabilitation programs. Loss of
balance and lack of coordination are main symptoms in stroke. Thus, patient James is unable to
coordinate in the rehabilitation programs. Loss of consciousness is another major reason that
create difficulty for the patients to actively participate in the program. In order to measure
cognitive function of the patient with stroke, medical professionals needs to apply Wechsler's
intelligence test. The major reason of applying this test is to test intelligence of the patients so
that treatment can be provided accordingly. This test will help the medical professionals in
identifying ability to perform cognitive function in the person.
