Introduction
Healthcare is one of the departments which demands the executives and the service providers
with accurate and specific qualities as a minor mistake or carelessness could lead to major
consequences and results. The working of the healthcare workers is quite sensitive and it
needs to be handled with specification and according to the models and theories specified in
the department (Gettel et al. 2019). I have gained some knowledge on nursing service
through my course and the practices I did which would be presented through the use of
Borton’s Development Framework. Borton’s Development Framework provides a clear
understanding of the experience and thus, the reflection is quite specific. For providing a
detailed acknowledgement of my experience as an intern in a renowned healthcare
organisation, Borton’s Development Framework would provide specification to each of the
factors to be considered in the reflection (Erickson, 2017). The experience would be provided
with specific dates on which the incidents took place as this would help in providing a clear
understanding of the experience. The three stages in the framework, that is what happened, so
what which specifies the results of the action and now what which provides the best solution
to the issue that was faced would be presented through Borton’s Framework.
Analysis of Journal (WHAT?)
The profession of nursing had been attracting me since childhood and when I took over the
profession on completing my studies, I realized the accuracy and the sensitivity that is to be
maintained and followed in this regard. While I was interning in a health organisation, there
occurred two prime incidents which helped me in gaining the knowledge about the serious
consequences that could be the outcome of minor carelessness of the nurse or of any health
care service providers.

Out of those two incidents I would explore one of them which was the major one. It was
between my internship, where an adult was involved. He was around 40-45 years of age and
he has been suffering from aesthetic asthma attack. He had diabetes’s and some other
common issues which needs to be considered while medicating a patient. He was my fifth
patient and I was being guided by the senior nurse while servicing him. My senior has been
quite accurate in her activities and she used to guide perfectly. Due to some emergency she
was on a leave for two days and this made me responsible for the patient and during those

two days, the patient got some serious asthmatic attacks due to which he was supposed to be
transferred to the Intensive Care Unit (ICU) during one night. I have been maintaining his all
the medical records specifically and through the process and procedures which was to be
followed. The transfer was so sudden that it made me a bit nervous and I panicked too as my
senior was not present with me. The doctor asked me about his various symptoms and other
considerations which I informed accurately, which was justified through the medical report
too. There is a process which is to be followed for the transfer of a patient and it requires a
complete informing report to be framed through specific models and theories. I was so
nervous due to the sudden shift of my patient that it led to multiple mistakes (Sloane et al.
2018). The transfer report which was to be framed through the process of SBAR approach
was not made accurately and it led to the miss-communication. I forgot to inform the doctor
about the diabetic issue of the patient as well as the neuron issue. My patient was not a
healthy one and he had multiple issues due to which I got confused and my nervousness
added on to the miscommunication problem. I informed the doctor about the childhood
problem of the patient but did not cross checked the patient’s chart while informing the
doctor, which would have helped in adding on to the information about the diabetes and
neuron issues.

I was so careless (to be raw in my language), that I did not follow the transfer procedure and
the note or the documentation that was required to be framed while transferring the patient
was not followed. While transferring the patient I just made a rough sketch of the patient’s
medical history where I missed multiple information. The process and the procedures along
with documentation and other activities nothing was followed by me, which created the
situation of serious health issue for the patient. The patient was provided with some
medicines and treatments on the basis of my report which worsen his situation and condition
for the next two days. His family members were not informed about the mistake that was
made, but it helped me in understanding the sensitive position that the health care takers hold.
Reflection (SO WHAT?)
The incident was scary and it could have impacted my career intensively but due to the
support and the proper guidance I was saved. The incident was a major lesson to me and it
helped me in realizing the impact and the role that a nurse has in a patient’s life and in the

health organisation. The position of each health care service providers is immensely sensitive
and maturity along with the accuracy is to be part of their personality.

When the condition of the patient started to deteriorate, and it went out of my control, that
made me nervous and thus, all the mess up began. The absence of my senior during that
situation, reduced my confidence and I panicked. On being interrogated by the doctor about
the medical history and patient’s chart, I provided all the information as far as I remembered
but missed few. This was the issue of my lack of communication skill. The other mistake was
that while making the transfer document, I didn’t followed any specific approach as taught
during the education degree, I messed up two to three approaches and thus, missed some of
the vital information.
While answering the doctor I actually messed up the entire information of the patient with the
thoughts of whether I am giving the correct information, is the information accurate, hope I
am not mismatching the information with other patient, am I not missing any information,
what if the situation has taken place because of me and multiple other questions were going
around in my mind while I was communicating with the doctor. Lack of confidence was the
major issue and this led to the miss-communication. I felt all has been my mistake which led
to the serious attack of the patient but it was actually not my fault initially, my mistakes got
added on as I started laming myself for the situation. The transfer documentation which was
supposed to be framed through some particular medical approach in order to include all the
details of the patient and his medical records, I ended up messing up two to three approaches
together and thus, no accurate information was provided to the doctors and the nurses. The
documentation of the transfer formalities is the basic step which provides the doctors and the
new nurse with the medical record of the patient and the treatment which has been given to
him. The base of the further treatment is the transfer report which was made inaccurately by
me due to lack of sensibility, lack of confidence, lack of accuracy, etc. The SOAP Note,
SBAR approach and few other helping tools of the medical degree all were messed up by me
in framing up the report and it created a new and some confusing medical record for the
patient.
Further treatment was based on the report which was framed by me and in the next few days
of the transfer, the patient condition deteriorated further which made me realize my mistake
when the doctor cross checked all the information on the report with my senior nurse. The

doctor scolded me and I was made guilty for the patient’s condition which I realized was
completely my mistake and it could have led me to some serious consequences which could
have ruined my career. Being an intern in the organisation, I was excused through
apologising and making the doctors and the authorities believe that it won’t be repeated. The
guidance of the senior nurse and doctors was to be taken on a serious note and the condition
of the patient was improving through consulting the senior doctors. When I was cross
questioned by the doctors it made me realize that lack of confidence and nervousness leads to
the scenario of adding on to our mistakes. A health caretaker needs to be calm and relax in all
the situations no matter how serious the condition is. The mental status should be calm and
accurate reasoning for each situation should be given.
Linking to Theory
As per Kerr et al. (2016), nursing is one of the sensitive and the responsible role which needs
to be played in a specific way or else it could lead to some serious and life taking
circumstances. The role of a nurse is to frame up the reports and the background of the
patient through various tools and techniques which is provided in healthcare which would be
followed by the doctor or the one who treats the patient. The personality trait of a nurse or
any health caretaker is as vital as the educational qualification of the same. The personality
traits needs to be well developed of the health service providers along with the education
qualifications and degrees to be gained by them. It is rightly said by Britton et al. (2019), that
the educational qualification of health caretakers or service providers is of no use if he/she
does not carry the personality traits of the profession. In health care sector personality traits
are as important as the educational degrees and each of the considerations should compliment
and align with each other.
As per Norouzi et al. (2018), the personality improvisation is an essential part of health sector
service providers but the management of the sector should also be quite active in their
approach. The Systems Theory of management is one of the approaches which is suitable for
the health sector as it helps in executing the activities through aligning all the other factors of
the management such as environment, culture, requirement, etc. The open systems
management carries a wide and a flawless approach towards the management which helps in
reducing the number of mistakes that could take place in the organisation. It has also been
mentioned y Carson et al. (2017), that human relations theory is another productive theory
specifically in the health sector which also aligns with the personality development

requirement. Thus, the health organisations should select its management approach in
specific and accurate manner so that the employees gets a better environment and workplace.
In the present scenario, the issue began because of lack of confidence and the messing up of
the information has led to some serious consequences. The personality of a health service
provider needs to be developed in accurate way so that the reactions and the decisions could
be made effectively and accurately. Any health care sector service provider needs to carry the
sensibility of considering the future consequences of his decision and then proceed with the
decision above this, he needs to have a good foresight so that the impact of the decision is
fruitful too. According to Carson et al. (2017), the nurse needs to be more active and specific
in her actions and decisions as she is the one who frames up the reports and the records of the
patient which is the base of the future treatment. The nurse has to be accurate in her actions
and she needs to consider the doctor’s perspective too in the absence of the doctor. In
emergency situations where the doctor couldn’t arrive, the nurse has to play his role and thus,
the responsibility of the nurse is dual in nature. The transfer of the patient is one of the
critical and the sensitive actins where the nurse needs to frame up the medical report of the
patient and provide the history of the treatment and medication so that the future medication
and treatment could be decided. As per Halvorsen et al. (2016), SOAP Note along with the
SBAR approach are among the most effective tools that could be utilised by a nurse for
framing up the transfer documents of the patient. The inclusion of the each and every
scenario of the patient’s situation makes these two documents framework quite impactful and
fruitful. In the present scenario too, had these two tools been utilised the situation would have
not gone against the patient as well as for the nurse. According to Norouzi et al. (2018), the
management theories if applied in the organisation then the working becomes much more
accurate and productive in nature.
The personality traits of a nurse or any health care service providers needs to be developed
and inculcated along with the educational qualifications. It has been stated by Norouzi et al.
(2018), that the personality of the health care service providers needs to developed and
checked along with the educational qualifications, as it is said a doctor with zero anger
management is nothing but a murderer with a knife in his hand. Anger management, mood
fluctuations, personal problems, lack of confidence, nervousness, frustrations all these are
common in human beings but a health care service provider has to demarcate these feelings
and thoughts when he or she is on duty. The medical institutes should provide the students
with the personality development techniques and traits along with the educational degrees in

entire situation in the current time has provided me with the perfect solution to such scenarios
which could help the early learners of nursing to reduce their mistakes. As I have experienced
the sensitivity of information that is actively present in the health organisation, it has
provided me with some additional and personal thoughts which is built on the basis of the
various authors and writers perspectives, in the medical field. On exploring the various
authors who has provided some authentic and effective models and tools for executing the
service in a n=more accurate manner, the following suggestions could be provided for the
present scenario which could have provided some effective results or could have managed the
situation in a more specific manner.
a. The foremost suggestion is that the medical educational institutions should provide
the personality development classes too, along with the educational degree. The
emphasis should also be put on the personality development or requirement of the
medical students as in the medical domain, the reactions and the behaviour of the
people health sector service providers plays a vital role in the execution of their
services. The institutes should provide the students with the practical knowledge too
on the control over their emotions and reactions as this would help in channelizing

their career to the correct goal. The institutes should provide scenarios where the
students get the practical knowledge of the reactions or behaviours they need to
execute in their profession with accuracy and specification (O'Rourke et al. 2018).
The present scenario could have been managed well if I would have gained this
personality traits knowledge during my course of nursing and thus, the scenario would
not have emerged up.
b. The second suggestion is that during the internship the guidance of the senior doctors
and nurses should be accurately provided as this would help in reducing the
probability of mistakes to take place. The framing up of the channel of
communication could help the organisation in maintaining the quality and flow of
communication. In the present scenario if the doctor would have accurately checked
the transfer report, the identification of the mismatching of the various approaches
and models in the report would have been made easily. This would have controlled
the situation in the best way. Even after a major mistake made by an intern without
her senior presence would have been acknowledged and thus, the patient would not
have to suffer.
c. The leadership of the organisation should be managed and planned in a way that the
leaders deliver accurate services and information to the juniors or interns. The
leadership should be selected through specification to the quality that is to be
maintained in the organisation.
The suggestions has been made in accordance to the scenario provided and t also carries a
long lasting result which could help in mitigating the impact of mistakes of the future interns.
Conclusion
On acknowledging the accuracy and the sensitivity that is to be maintained in the health care
sector, the significance and the requirement of the sector has also been explored. The health
care sector is along the most significant ones and it needs to be enhanced through taking up
the best and the latest technologies and tools. Nursing is one of the sensitive and the
responsible role which needs to be played in a specific way or else it could lead to some
serious and life taking circumstances. The role of a nurse is to frame up the reports and the
background of the patient through various tools and techniques which is provided in
healthcare which would be followed by the doctor or the one who treats the patient. The
