INTRODUCTION
In Health and Social care, nursing is generally a care profession that contains
concentration on the care of vulnerable patients, their families as well as the
communities so that they can effectively attain or can effectively recover the optimal
well-being and a quality of life. The profession generally involves an autonomous as
well as a collaborative care of the people of every age group, their respective families,
sick or being well across all settings. It can specifically involve the promotion of the well-
being, prevention of disease as well as the care of ill or disabled people. The care
nursing profession generally plays an essential role in order to treat the physical
ailments of the patient along with their emotional needs. In this, when the care nurses
can exhibit empathy, they specifically foster more collaborative relationship with the
vulnerable patient, that can aid in rooting out the specific causes, its associated
symptoms or the explanations which involve outcome in a proper diagnosis as well as
an appropriate treatment. The report will cover the roles and responsibilities which the
care nurses must follow in order to provide effective care services to the patients who
need it most. The report will cover a case scenario of Mr. Singh who was admitted in
hospital for his deteriorating condition. And this can be explained with the help of
reflective model such as Gibbs model of reflection (Kenny et. al., 2021).
MAIN BODY
The reflective model is generally a structured or an organized process which is
specifically used to guide the both personal and a situational analysis and improvement
as well. In this, reflection is generally a concept which generally emphasizes the
awareness of knowledge of an individual, their past experiences and beliefs as well.

The Gibbs reflective model is specifically based on various stages such as description,
feelings, evaluation, analysis, conclusion and an action plan (Choperena et. al., 2019).
Description:
There is a case scenario of Mr. Singh, who is admitted in the hospital for his
deteriorating condition. He is generally become ill and not feeling good from past few
days and due to this he is admitted in healthcare organization. There is needed an
effective care service which must be given to Mr. Singh such that he can effectively able
to improve his well-being. But the care nurse has not taken as well as recorded all of the
observations of the patient. As per the given case scenario, the care nurse has not
correctly calculated the subsequent MEWS (Modified Early Warning Signs) score. In
addition, the MEWS score generally enable the development in the safety and quality of
the care management team generally given to the surgical ward patients. its basic
purpose is to prevent the delay in the interventions or the transfer of the critically ill
patients. furthermore, here the role of the care nurse is to effectively monitor the patient
and can take the vital signs and symptoms of the patient so that on that basis, an
effective care plan can be made and followed as well. developing an effective care plan
can help both patient and care workers to efficiently improve the health of the patients
by providing quality care services as per patient’s need (Bracken-Scally et. al., 2021).
Feelings:
While providing the care services to the patient such as Mr. Singh, as a care
nurse, it is my duty to effectively evaluate all vital signs and symptoms of my patient so
that I can get to know what actually causes issues within their body. The body of the
patient become more lethargic and weaker such that he cannot able to perform his

activities such as not able to drink plenty of water. It is noted by the care nurse that the
blood pressure of the patient falls from normal and his heart rhythm generally fasten. As
a care nurse, I must record all the possible observations and behaviour of the patients.
I must calculate the MEWS score more appropriately so that on that basis I can further
decide to what step must be taken afterward. It is generally the care nurse’s
responsibilities to effectively communicate with patients, such that patients can feel
comfortable with care workers and can effectively able to develop trust as this thing can
make the care workers to provide an effective care as per the needs of the patients. In
this, the feelings here involve that the care nurses must effectively take care of their
patients as in the case of Mr. Singh, there were no care nurse available to take care of
him (Mackie et. al., 2021).
Evaluation:
It is specifically evaluated from the case scenario that there were a lot of negative
things that has happened such as not taking proper care of the patient, not taking his
vital signs and symptoms, not providing an effective care service to Mr. Singh, does not
provide enough intake of fluid to improve his conditions. All such factors are being
evaluated and are responsible for the Death of Mr. Singh (Sasser et. al., 2021).
Analysis:
It is analysed from the case scenario that as a care nurse, I must provide an
effective healthcare service to patients to improve their well-being more positively. First
of all, it is the care nurse responsibility to take all the possible vital sign and symptoms
of the patient so that nurses can make a care plan according to that and also can give
the quality care services as per Mr. Singh’s need (Greenberger, 2020).

Conclusion:
It is concluded from the above scenario that every patient must involve a right to
receive care services to improve their well-being more efficiently. The care nurses can
involve the evidence-based practices which must be used in their practice towards the
patient’s. but, in the case scenario, there was as such no evidence-based practices was
used to treat Mr. Singh and due to this, his conditions become more deteriorating and
ultimately causes death (Barbagallo, 2021).
Action plan:
The action plan here specifically required to develop; it is because it is quite
necessary to develop an action plan for care nurses to improve the health of the patient.
The action plan can involve an effective patient-centred care, an evidence-based
practices given to the patient who need it the most. In this, it is every care nurse
responsibility to effectively monitor every sign and symptoms of their patient so that it
cannot be happen again such as in case scenario of Mr. Singh. While providing a
patient-centred care and evidence-based practices to the patient’s, they must involve an
effective communication skill. As having effective communication skills in healthcare
generally make benefits as it can help to build trust of the patients which can make care
nurses to effectively foster the quality care services as per the needs of the patients.
The action plan can also involve the decision-making process such as care nurses must
involve the patients and their families while making decisions about their care plan. As it
is the right of every patient to get involve in their own care treatment plan (Bourne, et.
al., 2021).

CONCLUSION
From the above discussion, it is analysed that the role of the care nurses is quite
essential in the health and social care. While providing the care services to the patient,
they must involve the rights such as an informed choice and many more. In this, it is the
right of every patient to have knowledge of their care treatment plan. Through the
patient-centred care approach, the care nurses can able to deliver an effective quality
care services to the patient to improve their well-being. Having an ineffective delivery of
care services can lead to worsen the health of the patients more deteriorating such as in
case scenario of Mr. Singh. Delivering poor quality of care services and paying attention
towards his vital sign and symptoms, not record all his observations more appropriately
will led to ultimately death of Mr. Singh.
