INTRODUCTIONRenal disease is a kidney disease which appears as a progressive loss in kidney functionover a certain period. When the level of creatinine lowers the glomerular filtration rate, itdecreases capability of the kidney; hence it loses the capacity to excrete waste products. Thus, inthis respect the study has been describing pathophysiology of renal failure and along with thatsuitable nursing intervention are also discussed. CASESCENARIOThe study is based on a 66 years old lady who is having chronic renal disease because ofconstant intake of drugs. She also had lithium toxicity; hence she is also diagnosed with inducednephrotoxicity. From past 30 years, she is having lithium; thus this gave her renal disease. Thus,it can be said that the patient is having Chronic interstitial nephritis. Ability to observe and describing aspects of the case by relating the pathophysiology andpatient’s clinical conditionIn accordance with the given case, there are different type of issues that are being facedby the 66 years old lady. With this respect, one of the issue is related with long history ofdepression and that also for 35 years. A patient when they have long time depression, then thereare both mental and physical impact over the body. It is a type of issue that sticks individuals atany point of time and some they are able to overcome them but it is difficult to control. Further,there are different type of depression (Navarro, Ferreira and Nolasco, 2015). Among all thedepression, as per the given case, it is been 35 year since the patient is face issues related todepression. With this respect, Atypical depression is a type of depression which is hard to diagnoseand it lasts of years. In this depression there are some common symptoms which are decreasedappetite, increases in preference to sweets and chocolates, etc. There many reasons due todepression is caused, some of the reasons include genetic, personal trauma, high stress, etc.(Muriithi and et.al., 2014). As per the case given these type of issues can be faced by the patient.Apart from depression, due to long time of consumption of lithium toxicity which is a type ofmeditation that is taken when the patient has bipolar disorder (Robles and et.al., 2017). It is atype of disease related with brain disorder in which individual will feel unusual shifts for activitylevel, energy, moods, etc. In order to reduce the impact of this issue, patient was consuming3

lithium toxicity. Further, due to consumption of this medicine, it has caused individuals to getnegatively affected with Nephrotoxicity. This is a type of poison that is caused in kidneys. Thereare different type of forms and the drug that was being consumed have negative impact overrenal function and this can get affected in more than one way (Thomas and et.al., 2008). Further,this can be considered to be one of the most common type of kidney problem and it is generallycaused when the body is exposed to toxin or drugs. There are many negative impact on kidney,in this it includes body fails to making the body rid of wastes and urine. Moreover, in accordance with the analysis made by one of the doctors, it was identifiedthat renal function was down by 10% and it was found when normal testing was done of theblood. Further, it was identified that for about 30 years the patient was consuming Lithium andthis has caused to get affected with renal disease which is Chronic interstitial nephritis. As per the pathophysiology, Nephrotoxicite is used broadly to describe disease related tokidney (Walker, Hewitson and Becker, 2014). It has negative impact over proximal tubular cells.These are type of agents that are freely filtered taking up of proximal tubular epithelial cells andglomeruli. They are incorporated into lysosmes that happens with first interaction withphospgolipis. There are different type of critical factors due to which kidney injury occurs.Among the main reason that has caused patient with this issue due to raise regular consumptionof drugs (Navarro, Ferreira and Nolasco, 2015). When drugs are consumed, it is important to make sure that the time in which it isconsumed should be fixed when the time duration for the medication to be taken is not followedproperly, then it increases the risk of dose and it causes serious health related issues like one thathas caused to 66 year old lady. Further, Lithium is a type of drug for choice and is consumed inorder to overcome the issue of bipolar depression. As per the case, for the patient to overcomethis type of issue, she used to take high doses and this has caused her to face the problem ofkidney. In accordance with the clinical condition of the patient, it can be stated that there aredifferent type of symptoms that re faced by patient. In this context, few of them are as follows:Bring tired: A healthy kidney develops hormones which are known as erythropoietin(Muriithi and et.al., 2014).
