Introduction
In this assessment a digital project “My Health Record” project designs. This project is in Australia global
and local remote areas. The project provides the healthcare services to patients to book appointments of a
medical specialist, can get the medicines at low cost. Documentation for analysis of “My Health Record”
will be created.
A stakeholder map will be created for “My Health record” which includes four operations i.e. Internal-
Executive, Internal-Operation, External-Executive and External-Operation with their interest in the
project. After that, a questionnaire will be created for a stakeholder to find the further details and get the
feedback from stakeholders. In the last section of the report, I will develop the use case diagram which
includes use case description and full use case description.

Stakeholder Mapping
A digital project “My Health Record” is a health project which has the aim to deliver the quality services
to patients and an efficient information system which will contribute efficiency, feasibility, and stability
to the healthcare industry. Stakeholders are the type of person who contributes to an organization directly
or indirectly. Stakeholders play an important part in an organization as they can impact positively or
negatively to the organization. The stakeholders vary according to the type of organization in which they
work for e.g.customer, admin, manager, government authorities and many more other stakeholders. For
the digital project “My Health Record” the stakeholder analysis is performed in below section.

For the digital project “My Health Record” the stakeholder map is created which contains four different
sections that are “internal-executive, internal-operation, external-executive, and external-operation”. For
these operations, different stakeholders are associated. These stakeholders play a different role in the
organization and help to control the system of organization. From this stakeholder map of “My Health
Record,” a rough idea can get for digit health project and its operations (Smith, 2017).

activities like managing employees, staffs
and also to look the communication process
in the organization.
CEO Internal-operation CEO has the highest post in the
organization. His responsibility is to
manage the overall activities of an
organization like presenting new ideas and
methodologies.
Director Internal-operation Director is the person who is responsible to
take a final decision in the organization.
The technical staff and non-technical staff is
also managed by director only.
General Managers Internal-operation General manager works under the manager.
He is responsible to manage all the low-
level activities of the organization and
present report to the manager. The general
manager also responsible to conduct
training sessions for the new employees and
staff.
Transport service like
ambulance
External-executive The transport service like the ambulance is
used to carry patients and provides the
transportation service. In the digital project,
“My Health record” ambulance is the main
transportation service.
Medicine supplier External-executive Medicine supplier is the person who is
responsible to provide medicines to
patients. They are responsible to provide
medicines to the organization.
Government External-executive Government is responsible to incorporate
rules and policies to design the healthcare
project and also provides the facility of data
security and privacy.
Private company sponsor External-executive They are the person who invests in the
organization by providing funds.
Healthcare professional External-Operation They are the type of stakeholder who is
responsible to provide the medical treatment
facilities to patients of “My Health Record”.
Service provider External-Operation The service provider offers different
services like providing clothes, test and bed
and other medical healthcare requirements

Questionnaire
The questionnaire is defined as a set of questions that are proposed after analysis of the system. It is made
to fulfill the information gathering regarding the system. It is designed to identify the further details about
the environment system and how to solve the problems encountered. This questionnaire will provide to
the stakeholder. The questionnaire helps to identify the system performance. In the next section
questionnaire of “My Health Record” will be designed for stakeholder for finding the further details of
the project(McLeod, 2018).

Functional requirements of the system:
a. The “My Health Record” digital project provides global access to all its remote users.
b. It provides functionality to all its users to manage their medical prescription.
c. It provides functionality to manage their details they can update, delete their details.
d. Users can directly contact medical consultants and can raise their query.
e. The users can view more than one aspects of medical information over the time.

Conclusion
A digital project “Medical Health Record” is in Australia which provides the medical facilities to patients
in global and local areas. A report is also generated which aim was to gather the requirements of the
project. A stakeholder map is created for “My Health record” which includes four operations i.e. Internal-
Executive, Internal-Operation, External-Executive and External-Operation with their interest in the
project. For the betterment of project, a questionnaire is also designed for stakeholder map. In the last
section of the report, a use case diagram is created along with its description.
