Executive Summary
Safety and risk management is of the major responsibility, which is practiced by the
companies at every level to analyse the risk, weakness, and loopholes in the operation.
However, when such factors are ignored it might lead to major accidents and safety breaches.
Like in other industries, it is also essential in Railways where there is a high occurrence of
dangerous accidents. In the report, a brief description of the train accidents has been discussed,
flowing the factors, which has led to such disasters as well as measures taken by the government.
In this report, the discussion over the “Richmond Train Crash” has been conducted. Through the
medium of this report, readers can analyse the multiple factors which had led to such blunder
and what are the preventive measures taken by the government t to avoid the occurrence of such
major accidents.

Introduction
Today, practicing the method of safety and risk management in the workplace has
become critical for organizations to assist them to identify the unforeseen contingencies, which
are creating a negative impact on its operations and stability. Safety and Risk management can
be defined as a process of ascertaining the threats, measuring the hazards, evaluating and
monitoring that risk. (Miller, 2011) To become stable and maintain the market position, many
companies are providing special training to their workforce to eliminate the hazard and prevent
unwanted accidents, which can affect the system. It prepares them for uncertainties and dynamic
environment. Besides this National Safety Management Society has also stated that safety
management is a core element of managerial responsibilities as it highlights the degree of
commitment of the company towards employees. For the efficient and productive practices of
SRM, the company follows various principles (Bowman, 2015). It provides an action-plan to the
Company so that it can analyze the risk monitor the shortcomings of the management, which can
develop into major accidents and safety breach. A safety breach means misusing the powers to
conduct an unlawful act (Hopkins , 2009).
Like various other industry, the Rail transportation industry has adopted safety and risk
management technique to avoid the major accident and injuries in the operation, which is not
only hazardous to the passenger but also the company. Despite that mishap and collision happens
which are gruesome enough to claim the life of the passengers and railways operator (Australian
Tranport Safety Bureau, 2014). Such a safety breach can be occurred due to technical issues in
the system and because of negligence on the part of workers. Train operator inattentiveness, an
accident with other train n train derailment creates alarming situations, whose avoidance has

become critical for the government. The impacts of the safety breach which was happened in
Australia (Klockner & Toft, 2018).
Incident: Richmond Train Crash
In the beautiful morning of January 22, 2018, Sydney metro train Waratah- A42 was
departed from Central Point, Sydney to the reach the Richmond Station which is in Sydney
northwest smashed and collided into the end of the railway platform injuring 16 people. The
impact was so huge that the metro train not only crashed the railway crossing barriers but also
crushed the platform buffer stop (Deutrom, Clennell, & Ritchie, 2018). Due to which train
became of the track. All the 16 passengers were taken to the nearest hospitals. According to the
people who were present over there, described the incident as a whole mess and chaotic (Xie,
Hei, Mochizuki, Takahashi, & Nakamura, 2014). While describing the accident, they further said
that many travellers were moving and flying from one seat to another seat, when the train
became unstoppable and crashed around 9.50 am (ABC News, 2018). It was analysed, that the
speed of the train highly fluctuated and there were great differences in the speed before its
collision. In the whole incident, the train operator received minor injuries and the rail conductor
got some face and chest injurious. During the investigation, New South Wales Police indicated
that Sydney metro was striving hard to stop at the main station, however; its unstoppable nature
rammed and smashed the train into the stop (Filtness & Naweed, 2017). Many passengers
received fatal injurious in which 31year and 77-year women were hospitalized with a fractured
collarbone and 21year old adults with the fracture in the Femur (Magel, Mutton, Ekberg, &
Kapoor, 2016). They all were taken to the Westmead Hospital. Even on the social media people
have shown their grievance and condolence described the whole incident as disturbing and

horrifying where the pool of blood everywhere. Moreover, the manager of Sydney Train Howard
Collins in the interview said, “it was a pretty solid hit to the bumpers”. Though it was not clear
what caused the disaster, but after the inquiry it was discovered that unexperienced engineers
and lack of efficient operating system has led to catastrophe (Klockner & Toft, 2018).
Safety Breach
To investigate the reason behind such disaster, ATSB, which is an Australian Transport
Safety Bureau, was continuously investigating the accident case. However, there was not much
clarity about what had led to the accident (Too, Spittal, Bugeja, Milner, Stevenson, & Mcclure,
2015). It can be said that a lack of technical knowledge and skills and casual attitude of workers
while conducting the operations can be considered as reasons for incidents. ATSB, further
stated, that the team would also look into the history of the company, whether the efficient
installed operating system was there or not and other management factors. During the
investigation, investigators conducted the medical a test over the driver and in which it was
analyzed that he was medically fit as well as the barriers were free of pollutants (Australian
Tranport Safety Bureau, 2014). While interrogating the driver, it was revealed that the driver was
well experienced and was attentive. The report published by them also claimed that everything
was clear and there were no problems with the signal issues. Bureau had seized the private
documents of the company (Zeigler, Paul, & Monastero, 2016). After the research, analyzed that
the train lacked computerized rail protection system and due to the human error such adverse
events caused. This disastrous event could have been avoided by installing an automatic train
protection system’ in the controlling system of the company. As per the study of News Crop,
government officials failed to manage “the automatic train protection safety system” which

controls and monitors the speed of the trains at every level. ATP controls the speed of the train
when it is exceeding its original speed by putting an emergency brake (Xie, Hei, Mochizuki,
Takahashi, & Nakamura, 2014).
The aftermath of the incident
After the frightening and disastrous incident, Richmond station was shut down until the
investigation. OTSI took the command and investigated ATSB, which further said that there is a
need for a safe working environment and exposure to the workers regarding all types of
uncertainties and challenging situations (Gemma, Lenne, & Moss, 2012). NSW (New South
Wales), transport minister Andrew Constance stated that the industrial discussions between the
Sydney train and government delegates would be conducted after the temporary closure of the
railways to ensure the interest of the passengers by compensating them (ABC News, 2018).
Policy and Implementation
After the heart-wrenching incident, the government has introduced reformed to prevent
train accidents in the future. Moreover, public transport Authority of Western Australia will
launch Alstom Transport Australia to enrich the experience of the passengers by providing
advance services through new technology operations so about 1200 passengers can reach their
destination safely (Reportable and recordable, 2013). According to the rules of ATSB, workers
must be well qualified to take the responsibility of the passengers. They must be prepared for
future contingencies (Railway Gazette, 2019). In 2017-2018 budgets, the Australian government
launched 10 billion National Rail Programs as an investment to provide the best experiences
while focusing on the safety and security of the travelers. It has been done to upgrade the
infrastructure and conditions of the railway, which also involves an efficient system of

operations. In the following year that is 2019-20, the government has distributed $3.5 billion to
the Western Sydney North Rail Link and $2 Billion for Geelong and Melbourne (Railway
Technology, 2017).
Conclusion
It can be inferred that safety and management is an essential technique of monitoring the
hazards, which can affect the stability of the company. This process helps the company
analyzing risk and plan the strategies accordingly. However, went they are not followed it might
develop in major accidents and safety breaches. Such accidents can create a long-lasting impact
on organizations. In the report, the focus is over the safety and risk management in the railway
industry of Australia, wherewith the help of train accidents (Richmond Train accident) kind of
safety breach has been discussed. Besides this, the policy has been discussed.
