Executive Summary
In this report, the safety and risk management principles and procedures are discussed. A case
study of a train accident which occurred recently in Kyogle, New South Wales has been
depicted. This case study can help in analyzing the breaches of safety measures which led to the
accident. The policies and procedures that the government needs to change to avoid future
accidents have been reviewed. The procedures for implementation of such policy are discussed.
