 Health care provision being complex in nature is a difficult task
in itself and a further more typical disorder makes it even more
complex task to complete. With a more complex disorders
comes a even more complex needs of the service users of a
health care setting.
 In case of dementia, the care needs of the patient elevates to
requirement of support in performing even the basic daily
routine activities making it necessary for building a proper care
plan for the patient.

Mrs. E' s son includes episodes of confusion while performing

contribute towards the health such as social, cultural, political
and environment.
 Holistic model: With occurrence of holistic model, it is
typically considering the whole person body mind and spirit.

 The major of time Mrs. E is alone in home and have no social
background. In addition to this, the social model are usually
taking under the consideration which may create positive
impact on the Mrs. E health.

where people used to taking into the participation where some
of the people are used to taken under the process.

Conclusion
 From the above report it can be concluded that for taking care
of a dementia patient a proper care plan needs to be formulated
in order to meet the care needs of the patient. Implementation
is also important for the success of the formulated care plan
and in the success of a plan various health care professionals
play important role. In addition to that good communication
skills also plays an important role.
