Instructions
These instructions must be read in conjunction with the Unit Outline.
Cheating and plagiarism is considered academic misconduct and will not be
tolerated at WAI. Refer to ‘Cheating, Plagiarism and Collusion’ statement in
the current ‘Student A to Z Guide’.
You need to demonstrate what you have learnt during this unit by
completing the following written questions. This assessment will contribute
towards the assessment of your overall competency.
This Assessment Task requires you to answer a series of short answer in a
written format, using the space provided for each question or statement, to
demonstrate an understanding of the skills and knowledge required for
implementing and monitoring work health and safety practices.
OR
This assessment activity requires you to answer a series of oral questions.
The assessor will record your answers in a written format, using the space
provided for each question or statement.
Instructions for a written assessment:
You must write clearly and answer all sections.
A black or blue pen must be used if in a written format,
otherwise you may use word processing.
Assessments written in pencil or red pen will not be accepted.
White out or correction fluid / tape is not allowed on
assessments.
All written responses must be in English.
If you need to make changes you must cross out and initial the
changes.
If you are unsure of any question then please ask the trainer for
clarification.
The ‘Student Portfolio Checklist’ is to be used during this
assessment task. Please ensure that you review this checklist prior to
undertaking the assessment task. If you are unsure of any task or
question please seek clarification from your trainer.

Each student will require a copy of the questions, using the following
handout.
Students will require a black or blue pen. Pencils or red pens are not
acceptable.
Access to workplace files, such as Policies and Procedures.
Access to equipment and their supporting manufacturer’s
instructions.
Access to relevant Legislation, Compliance Codes, Codes of
Practice, Regulations, Standards, etc.
Context and
conditions for
assessment
Classroom or simulated work environment.
Approximately 1.5 hours to complete (including 5 minutes read
through). Timeframe can be negotiated based upon student’s
individual needs and requirements.
This is an open book Work Project, completed in groups (maximum
of 4).

Reasonable
adjustment
Assessors can apply reasonable adjustment to assessments. Reasonable
adjustment is the process of adjusting or changing the assessment to meet
the needs and characteristics of the student(s) being assessed, and taking
into account any equity requirements.
Identify level of assistance permitted (if any). Assessors can refer to the
following policies and procedure:
Support for Students with a Diagnosed Disability Policy
Diversity, Access and Equity for Students Policy
Cheating, Plagiarism and Collusion’ statement in the current
‘Student A to Z Guide’.
Provide justification that the alternative procedure for collecting candidate
evidence does not impact on the standard expected by the workplace, as
expressed by the relevant unit of competency
For example:
Student is provided an extension of time.
Student is provided with verbal assistance to interpret the question.
If a reasonable adjustment has been granted to an individual student, record
this adjustment on the recording sheet for that activity.

Standard
To be assessed as satisfactory for this assessment activity each
question must have the correct response.
Successful completion of the assessment activity requires the
student to submit the required pieces of evidence that demonstrate
the performance standard outlined. Criteria will be ticked off as
satisfactory when the assessor has reviewed the evidence provided
against the Portfolio checklist.
Reassessment
Reassessment and appeal information is to be read in conjunction with the
Unit Outline.
Reassessment
If you are not marked satisfactory on all criteria in the ‘Assessor
Portfolio Checklist’, your assessor will arrange another time to
resubmit evidence. The trainer will provide guidance on their copy of
the ‘Assessment Tasks Grade Sheet’ as to the areas of the
learning materials that the student needs to review prior to the
reassessment.
Students who do not achieve competency in a unit after the 2nd
attempt must re-enrol in the unit in the following teaching period.
Students, who are required to re-enrol in a unit which they have
previously not been successful in completing, are required to pay an
additional tuition fee.
Assessment Appeals
If you wish to appeal an assessment decision you should initially talk with
your trainer/assessor and see if the situation can be resolved. If you are not
satisfied with the outcome you may then lodge a formal complaint.
