Value based health care is a new approach and model that is used by various hospitals
and physicians in order to increase the value rather than volume. This study will show about the
importance of value based care for its services users like patients, suppliers, providers, payers
and society. Fro improving value based health care many doctors and staff use development plan
and then follow that plan. The main aim of making that plan is to improve their skills and
provide the better facilities to patients by eliminating and reducing the weaknesses of themselves

health also reduces the need of expensive procedures, tests and medicines. In value based health

There are various importance of value based healthcare for suppliers, patients, providers and
society.
Patients: In value based health care, patient paid and invest less money in order to
achieve better and improved health. Some chronic diseases like diabetes, cancer take a lot of
time and money of patients. Value based model concentrates on helping patients in order to
recover them from illnesses and injuries and also avoid chronic disease at a very first stage. It is
beneficial for patients because in this type patient need to face fewer doctor's visit, procedures
and also have to spe4nd less money on medication (Flott and Linden, 2016).
Providers: With value based health care, providers receive efficiencies and the highest
patent satisfaction. They also spend less time on chronic diseases management. When they focus
on value rather than volume then patient engagement and quality of patients measures increases.

reduce risks. By spreading this health care among a larger patient population it can reduce risks.
Value based payment allows payers to maximize their efficiencies by bundling payment which
covers the patient's chronic conditions and also cover full care cycle.
A singular language: Value based healthcare focus on providing a singular language that
is used and comprehended by medical teams, doctors, patients and their families. Value of
patients can be defined by an equation (Silverman, Kurtz and Draper, 2016).

preventive diseases and also treat them from chronic conditions. In order to care coordination ,
value based care providers focuses on new technologies which access to financial incentives and
data. In value based care data are analysed in order to identify health risks and according to that
improve care coordination and also improve organisation's operations. In terms of costs, values
are the new key standard for insurance companies and in value based care, the pay is based on
the quality and improvement in patient's health.
The main focus and aim of value based health care is on data that is measurable, patients
care, coordination of services and communication collaboration. Changing lifestyle of people are
majorly contributing the changes in the health of the world population. In many countries, the
cost of healthcare is also rising rapidly as they spend up to 10% of their GDP on healthcare. It
focuses on value more than volume that is key element in healthcare system.
A link between outcomes and health care costs they need to be established for delivering the
higher quality care for patients which can also help healthcare providers in order to manage cost
increases (Kullolli and et.al., 2017).
There is a difference between volume based healthcare and value based health care.
Volume based healthcare model provides measurements which help them in evaluating care
according to numbers of patients that are treated. On the other hand in vale based healthcare
model puts the overall needs of overall populations and individual at the main centre of the
picture.
It is very important and impact on the people and providers. It removes obstacles which
many patients and people are facing now. Many people and health care providers face many
problems that involve and related new care models. For example, A well known hospital has
recently adopted a telehealth model that provides remote oversight of patients with multiple
health diseases and problems at home. This telehealth model and solution help to reduce
hospitalisation rates.

For improving and making successful to value based care it can build a strong data
foundation. The right data foundation helps them in accessing their current situations and also
identify their opportunities for improving performance. It can also reduce the variation in
performance and cost. By reducing variations it can help them in comparing performance
between physicians and facilities. It can also introduce value by considering all aspects of care
that affects patients. It can also evaluate and measure values dividing quality by cost which is the
relevant aspect of quality against the cost which is providing care.
For improving the total cost of care it can total and count all the medical expenditures for
group of individuals and patient. The ability of providers to manage total cost of care is
important for remaining profitable under competitive market. Sustainability of vale based care is
also important for improving quality. Many quality projects fail after they meet their first and
initial goals. Manager also shift their attention towards new goals when they achieve their initial
goals. For maintaining and improving value based care it is important for them to make a solid
framework for sustainability (Lloyd, Bor and Noble, 2018).

Introduction: Personal development plan is a structured process that is undertaken by an
individual in order to reflect their own performance, learning and also to plan for their personal
and career development. The main aim of nursing development plan is to improve their learning
that can help them in maintaining skills and building knowledge to provide competent and safe
services and care to their patients.
It also support fro growth and career development to employers by which they can
increase engagement and decrease the turnover of nurses within an organisation. By investing in
personal professional development plan, nurses can influence patient experience and can target
their learning goals which helps them in progressing and increasing in their career wherever they
are in their career (Macchi and Kessler, 2018).
There are some tips by which they can improve their professional development plan:
 By investing in professional development and then make development plan. It can set
time in order to review and analyse their plan for accomplishing goals.

 After making plan they can ask to their employers that what types of development plans
and programs are available for them.
 By using reflective practices it can identify their learning needs. After that it can improve
their learning and experience.
 It can also discuss their learning goals and needs with their nursing leaders and their
mentor.
Personal vision: As a nurse practitioner, my personal vision is to make lifelong
relationship with patients by providing them safe, holistic and sensitive care. For achieving this
vision I need to improve creative use of knowledge, technologies, time management and ethics
towards patients.
Ideal future job role: Ideal job role for future is to make and become nurse anaesthetist
and want to work with National health service for needy people.
SWOT analysis: There are some strengths, weaknesses, opportunities and threats of
nurse that need to be develop and use in their practices. It is very useful and important method
that help them in identifying their strengths and weakness related to their purpose that need to be
improved (McWalter and et.al., 2015).

and supported thoroughly and effectively. Communication is another main issue that create
barrier and influence action plan. For reducing risks and for better management of risks they
need to examine certain possibilities that may affect the process and action plan. They can
evaluate and monitor the roles of specific members by which they can monitor risks throughout.
Importance of action plan
Producing and developing an action plan and personal development plan is beneficial for
not only staff members but also for clinics and hospitals (Reece and Walker, 2016). It allows
staff member and doctors to monitor and control their progress and then take necessary steps for
improving essential skills. The main advantage and goal of making action plan is it allows them
to execute a structured plan in order to achieve goals. It also creates a strong bond and
relationship with staff as each member of the hospital is aware of their role which help them in
providing necessary informations to ensure success of the project.
CONCLUSION
From the above study it has been concluded that value based healthcare played a vital
role in health care and hospitals. It focused on the value of outcomes rather than focusing on
number of patients treats. It also showed the importance of professional personal development
plan in which it showed their vision, strengths, weakness, opportunities and smart objectives that
need to be achieved for satisfying patients. Strengths and opportunities helped doctors and staff
in improving value based care and sustainability of that. It has showed the importance of action
plans and also the factors and risks that influenced that action plans and became the barrier in the
growth of their career.
