Part 3
Tables for quality management
The patient falls from the wheel chair while waiting at the waiting area.
Management causation scale
The clinical care evaluation regarding the clinical care by rating of the evidences regarding
the presence unfavorable conditions due to the medical management. (The highlighted
ones are selected.)
1 Little or no evidence of an adverse event caused by the management.
2 Slight evidence.
3 Not quite likely ( less than 50:50 odds but a close call)
4 Most likely than not (greater than 50:50 odds but a close call)
5 Strong evidence
6 Virtually certain evidence.
If an adverse event has occurred, where did it occur? (The selected options are
highlighted.)
1 Within the hospital/ imaging facility
2 Another hospital

relevant clinical details will be given.

SONOGRAPHIC ENVIRONMENT
From the previously mentioned table, it can be concluded that this kind of adverse event took
place because of the carelessness of the hospital authorities. Hence, there is a strong evidence
that this adverse event took place because of the faults of the medical management (5) within the
hospital/imaging facility (1).
Medical management severity scale
Coding of the severity of the unfavorable events and the selected one
0 Minor severity No disability, no significant
resultant discomfort of
functional impairment; and
no increased length of stay
as a result of the adverse
event.
1 Minor temporary Minimal to moderate clinical
effect requiring no or
minimal clinical
intervention, or no increased
length of stay or re-
presentation for the same or
related problem.
2 Minor permanent Minimal to moderate clinical
effect with permanent

residual and without
significant functional or
cosmetic impairment.
3 Major temporary Moderate to severe clinical
effect with no significant
functional cosmetic residual
effect. This usually results in
increased length of stay or
re-hospitalization and
requires moderate to major
clinical intervention.
4 Major permanent Moderate to severe clinical
effect with significant
functional or cosmetic
residual.
5 Major continuing or
potential major
When doubts exist as to the
outcome but the probability
is that a major impairment or
repeated presentations or
hospitalization will be
necessary. The outcome may
result in major impairment.
6 Death

SONOGRAPHIC ENVIRONMENT
From the previously mentioned table, it can be concluded that the rating of the medical
management severity scale is minor severity. After falling from the wheelchair the patient got
hurt but not that much that their stay in the hospital would increase. Along with this, no
disability or discomfort is faced by the patient after falling from the wheelchair.
Error causation scale
Identification of the probable causes of the unfavorable events
T1 Complications/ failure in technical performance
T2 Failure to follow policy, protocol or guideline when indicated.
T3 Acting on sufficient information.
T4 Slip/ lapse in activities in which operator is skilled.
T5 Inadequate knowledge/ experience.
T6 Inappropriate or not indicated care.
T7 No/ wrong / delayed treatment
D1 Failure synthesis/ decide/ act on available information
D2 Failure request/ arrange procedure or consultation
D3 Missed diagnosis
S1 Lack of care or attention/ failure to attend when called
