INTRODUCTION
Health and social care support is an integral task which is very important to be provided
to individuals who are subjected to mental or physical illness. Furthermore, it is crucial for health
and social care organisations to support people in ways which fulfil their health needs (Thomas,
2017). The following report is based on analysis of health and social care needs available to
individual requiring multidisciplinary care. It involves local community resources and provision
that support integrated care planning and local unmet needs related to healthcare provision in
own locale. It also includes difference between health and social care providers and types of
inter-agency care provision along with evaluation of local resources.
P1. Local Community resources and provision supporting integrated care planning
Integrated care planning is an appropriate and structured practice which is associated with
provision of health and social support to individuals who are suffering from specific clinical
problems (McCormack and McCance, 2016). To support the same, it is very important that
certain local community resources are also present and be utilised to provide integrated plan to
such individuals. Within London Borough of Brent, there are several local community resources
which are mentioned below: Care Home: One of the most effective resource which is present within the community
for provision of care of patients is care home. It provides distinct services like social
support, medical care and even non-medical facility for living which allows individuals
like Jonathan to appropriately acquire effective health and social services. Financial Assistance Organisation: Another appropriate local community resource is
organisations which provides financial assistance to individuals who could not support
their own living.
 Cultural Organisation: This local community resources deals with cultural aspects and
provides regional services as well. Moreover, these organisations have several volunteers
aiming at providing effective services to people suffering from health issues, for free.
P2. Local unmet needs related to healthcare provision in own locale
Despite having effective community resources, there are still several unmet needs which
were related to healthcare provisions within the organisation. In context with Jonathan and
London Borough of Brent, certain local unmet needs are discussed below:

 The first need which is unmet in relation to healthcare provision in the locality is shortage
of healthcare professional to monitor medical needs of individuals. People like Jonathan
require effective and essential assistance regarding their medications and daily
movements which could only be possible by presence of medical professionals (Åhlfeldt
and et. al., 2016).
 Another unmet need within the locality is shortage of emergency services. Seeing the
case of Jonathan where unpleasant smell is experienced by the neighbours, it is evident
that there are not routine round-ups or nearby emergency services to take care of the
same.
M1. Difference between health and social care providers and types of inter-agency care provision
Healthcare as well as social care are quite different in both context as well as application.
Furthermore, it is very important that differences in providers are outlined which enables
individuals to approach the right professional for their health and social care needs. There is a
prominent differentiation within these providers which is better displayed below:

 Financial Resources: There are various organisations such as NHS which partner up
with effective organisations to provide financial assistance to individual seeking medical
help. This allows these individuals to support their treatment and effectively allow them
to carry their treatment smoothly.
 Informational Resources: Another effective aspect on which NHS is working with other
organisations is to implement an information system which provides access to data
related to patients requiring multidisciplinary care. This has allowed the organisation in
providing care and support to individuals like Jonathan.
D1. Evaluation of Local Resources in meeting needs of individuals
Within health and social care, it is very important to evaluate the local resources to
determine how effective they are in relation to providing effective health and social care needs to
individuals like Jonathan. This evaluation is mentioned below: Care Home: This resource is quite effective in meeting needs of individuals as a care
home in locality would provide an ease to patients to acquire effective health and social
care services. In addition to this, it is also very necessary to ensure that patients are not
dependant on anyone for their needs. Financial Assistance Organisation: Major issue which arises in healthcare with people
like Jonathan is that these people are dependant on other individuals for effectively
managing their finances so that they could support their healthcare. However, financial
assistance organisation could be quite appropriate in operating in an effective way that
would allow patients like Jonathan to manage their finances effectively.
 Cultural Organisation: As per the case, Jonathan used to spend time in a local pub and
now with his degrading condition and neighbour being in hospital, he is somewhat
limited to his own house. Having a cultural organisation would help the man in being
socially active and could appropriately acquired social support (Crane, Lepicki and
Knudsen, 2016).
CONCLUSION
Thus, it is concluded, that it is very crucial for health and social care organisation to
provide multidisciplinary care to individual suffering from illnesses. It is essential to identify and
evaluate local community resources which would help in supporting integrated care planning.

Further, it is also important to determine local unmet needs which allow individuals to
effectively analyse and plan acquisition of resources which could fulfil the same. Lastly, it is
crucial to evaluate difference between health and social care providers which effectively allows
to understand interagency care provisions appropriately.
