INTRODUCTION
Integrated care refers as a way of coordinating health and social care services in respect
to make sure that they are closely depend around individual's needs. The proper integration of
health and social care sub sectors is essential in regards of being able to provide services for the
well-being of people and to meet the increasing demand of a growing and ageing population
through developing complex preferences and needs. The key motive of health and social care is
to providing effective health care benefits to the needy people in respect to making their living
effective and comfortable. NHS is the chosen organisation for this particular report, it is a large
health care services provider firm. The company has started its operations by the combined
efforts of NHS in England, NHS Scotland, NHS Wales and the affiliated Health and Social Care
in Northern Ireland in the year of 1948. The key motive of NHS is to providing effective health
care facilities to the individual who are facing any kind of issues and conflicts related to the
health condition. This report contains the study about local resources that support integrated care
working, need related to the health care and support system, an effective support system that
support service setting towards the identification of their own care needs. Here, the role of health
care or support service practitioner for supporting person concerted care, including proper
leadership within the remit of own role in a health care also described. Along with the proper
responsibilities of information sharing among multidisciplinary teams will also discussed and the
different communication methods applied to provide suitable support to different individuals and
appropriate strategies of communication will also discussed in proper manner (McCormack and
McCance, 2016).

P1) The local resources and provision that supports integrated care working.
Integrated care refers as an approach to strengthen an individual's centred health systems
by the substance of the comprehensive delivery of effective qualitative services across the life
course. Integrated care brings together the multiple groups that are included in patient care, Thus
in this patient's perspective, the service delivered are consistent or coordinated. It is basically
designed to the multidimensional perspectives of the population and the people or delivered
through a coordinated support of people. Here, types of inter-agency care provision to meet

needs of individual care also depend on type of care providers, i.e. health-care and social-care.
The difference between these two sectors can be analysed in following way -
Health-care Social-care
It includes needs related to treatment, control
as well as prevention of a diseases, injury or
illness.

the people to manage complex relationship

It emphasis more on provision of health-care

Financial Resources: This considered as the key aspect that effectively support care
working, thus it is essential for an integrated care working to maintain appropriate financial
budget in respect to execution of all business activities in proper manner. In the context of health
care, appropriate amount of fund is required for smooth implementation of organisational
functions and activities, thus proper finance is effective in making arrangement of proper
equipment, resources and also plays vital role in conducting all health care practices in proper
manner.
Human Resources: It is also an another key aspect thus it is effective in providing
support to integrated care. The key consideration of health care organisations is to maintain a
proper flow of workforces that are effective in providing support to the organisational operations.
In the context of health care organisation, proper human resources like doctors, surgeon's, care
service provider, nurses etc. are requires to have by the care homes for maintaining proper flow
of services. Here, nurses help in providing effective care to patients for their well-being, while
doctors focuses on giving appropriate treatment for increasing life-expectancy of them. Along
with this, managers concern on availability of resources and manage workplace so that better
services can be provided.
Equipment and Technologies: In today's modern era human life is very much based on
technological equipment and tools, thus these are effective in making humans life more easy and

comfortable. In the health care sector, organisations are widely concern over using advance
technology and medical equipment in respect to providing effective patient care facilities that
effective helps the doctors and professionals in properly diagnosing the diseases and treat
patients effectively and efficiently. In health care organisations, advancement in technologies are
high as health care organisations are using high-tech equipment like SPECT/CT, LINAC
Accelerator, High Dose Rate Brachytherapy, Digitalized X-Ray, USG, CT-Scan, MRI, ECG etc.
in respect to providing effective facilities to the patients. Here, for welfare of older people at
home-based services, NHS provide wheel chair and mobility scooter to cater needs of them.
Health care organisations are based on some provisions which are effective in providing
support to the integrated care working. It is essential for the health care organisations to follow
ethics and policies which are implemented by the health care provisions in respect to smooth
flow of operations (Brooker and Latham, 2015). Health care firm requires to prepare an effective
time schedule in which suitable professionals are appointed to provide appropriate facilities to
the patients. Organisations also needs to be attentive towards strengthening the workforce by
hiring skilled and talented professionals and surgeon. Integrated care firms are required to
implement effective collaboration in between of them in respect to conducting proper operations
and functions in effective manner without facing any kind of issue and conflicts. NHS is one of
the largest health care services provider which works under the health care ethics and per the
legislations and norms for providing well-being and effective care facilities to the people.
P2 Current local unmet need related to health, care and support service provision
In order to integrated health and social care needs, it is essential for management of NHS to
identify current unmet needs of people. This would help in integrating better support service
provision for well-being of people. It has been evaluated from a survey that older women in East
London are leaving health care or community centres of NHS, that offered service provisions of
older people, to live independent and healthy life, because service provided by health
practitioners of this organisation would be less as per expectation. Older people in England are
facing considerably a number of challenges in regarding with accessing the social services which
are essential for them to live independent and dignified lives. As per the interviews conducted in
2017 and 2018, with local people of old age with Human Rights Watch, it has been observed that
older people have expressed highly concerns about how local officials at home care community
centres determined their eligibility in terms of social support (Unmet needs, 2019). This kind of

services include generally in-home assistance, for instance - preparing meals, dressing, and
bathing etc. Along with this, in many cases, older people have denied to get crucial services like
emotional support, social interaction etc. that are essential for their well-being under the Human
Rights Act.
Community centres are mainly established within local area with purpose to provide health,
care and support services to older people, who are unable to get easy livelihood. But via survey
in 2018-19, it has observed that older people have made complaint against care takers who were
not understand their disabilities as well as social care needs (Unmet needs, 2019). According to
England’s Care Act 2014, any people who meets financial as well as needs criteria, must entitled
to government-supported social support, which is known as social care (Oliviere and Hargreaves,
2017). Such provision of services are mainly aim to promote independence and well-being, with
health and safety, via “supporting older people at community or health centres to live as
independently for as long as possible.” But apart from such provisions, such centres are failed to
provide effective care to older people, as they have a large number of complaints against
community centres with Local Government and Social Care of UK. According to some social
expert report with regards to social care assessments, serious concerns related with assessments
and mechanisms are concerned as serious to ensure their quality. Many people have made
complaint against care takers that their relatives who had been lived with such centres, were died
because of not getting proper services. It includes social care assessments, problems with entire
system and policies, delay in services, etc.
Therefore, in order to promote health and well-being of people especially older ones, it
becomes essential for NHS to open home care centres at local areas of East London, with
effective provisions of support and care services. This would help older people to get proper
support for living sustainable and independent life as much as possible.

needs
Under the United Nations Convention related to Rights of Persons with Disabilities
(CRPD), it has been evaluated that every people whether disabled or older ones, have equal right

to live independently within a community or home-based care services. To live a normal and
sustainable life after an age, they have right to get support for living independently, which
include services such as access to a range of in-home, community support and other residential
services, including personal assistance which is essential to support living and inclusion within
community for prevention from isolation or segregation. Here, right to live independently or
being included in home care centres are not depend only on the level of support that a person
needs. But, it depends to respond towards specific social care needs for people having
disabilities, defined as individuals having long-term physical, intellectual, mental or sensory
impairments, which in turn increase barriers to live independently and equally as others.
Therefore, concerning on needs of such people who are older or disabled ones, it is essential for
NHS to increase accessibility of community health care centres for them. It has to be provided
effective services to these people regarding with their actual needs. Community hospitals must
offer special beds which are not designated to speciality but concerned with specific needs of
disabled or older people (Brown, 2016). According to Care Act 2014, which has made provision
of care and support the people especially older ones, mainly concerns on promoting individual
well-being, preventing needs for care and support, promoting diversity and quality in provision
of services etc. Therefore, NHS must integrate services regarding with people admitted for
inpatient care like assessment, rehabilitation, sub-acute or end of life care and more, for
providing best care to patients. Along with this, practitioners must require to give support to
older people a range of clinics, which includes rapid access clinics, rheumatology, audiology,
mental health services and so on. It is essential for NHS supporters to concern on three main key
characteristics while offering community health care services. It includes local medical care,
support from local communities and integrated working. Hereby, availability of local family
doctors is considered as main feature of community hospital care who are expert in offering
intermediate care. Similarly, via enactment of Mental Capacity Act 2005, it assists NHS that to
make any decision on the behave of people who lacks capacity for the same, must concern on
their best interest. It includes decision for major medical treatment for increasing life-expectancy
of such patients. They are also acts as a bridge between primary care and secondary care
supports for well-being of people. In addition to this, NHS must provide a multi-disciplinary
team in community centres which includes a team of nurses, social workers. physiotherapists,

P4 Role of health, care or support service practitioner in supporting person-centred care
Nurses plays an important contributory role within the multidisciplinary team with regards
to approach to patient care. They are highly responsible for providing care to meet specific needs
of patients during the critical phases to live a normal life. As community based health care
centres offer services as per basis of person-centred where actual needs of community people are
concerned, therefore, in offering support with health, care services, medical practitioners and
care takers are played the main role. They give highly support to disabled ones and older people
in living a sustainable and independent life as much as possible. For this purpose, National
Health Service (NHS) supports its staff and authority members that are providing community
health, care services, via developing a network of team members who are able for sharing good
practice, offering appropriate service and promoting community hospitals. At community
centres, nurses, social workers, physiotherapists, occupational therapists and more, are working
in collaborations to support people for progression of their recovering from disability, an illness,
specific condition, an incident or intervention such as major operation, who need extra support
and care to live sustainable life. In this regard, nurses play the main role in providing a wide
range of services to people in retaining and maintaining their independence. They also give
support to those people at person-centred care who are living with long term conditions such as
deteriorating situations, life threatening illnesses, patients having special needs for palliative care
and more. There kinds of responsibilities are considered as major challenge for medical
practitioner’ team with such a wide remit. This would require a wide range of skills, therefore,
NHS also provides specific clinical support and training to nurses so that, they can provide
effective care and support to local people for their well-beings (Kogan, Wilber and Mosqueda,
2016). In community service setting, a primary role of nursing-care delivery system is to
enhances the role of nurses in supporting with goal of patient well-being and recovery.

P5 Appropriate leadership within the remit of own role in a health, care or support service to
promote effective inter-professional and multidisciplinary team-working
In order to provide effective care services and support to old people and disabled ones at
health care centres, as a manager in healthcare organisation, I concern more on developing new
strategies and modify ways of working. This would lead to a greater degree of inter-professional
collaboration between multi-disciplinary team members. Focusing on shortage of person-centred
care practices, with growing need for support and care services, as a manager it is essential for
me to bring new competences at care centres. It includes new ways of collaboration among
professionals, requirements as well as expectation of people to get effective services and more.
According to the Framework for Action on Inter-professional Education and Collaborative
Practice, World Health Organisation described inter-professional collaboration at health care
centres as the innovative way to meet the complex service requirements of old age people.
Therefore, to address all such specific needs, adequate and effective leadership required which
helps in bringing collaboration among multidisciplinary team-members. In this regard, I also
concern on adopting the effective style of leadership, where I have concerned on three main
areas, that helps in developing effective collaboration among team-members. It includes – self-
governance, hierarchical-governance and co-governance (Kogan, Wilber and Mosqueda, 2016).
Self-governance defines as the individual’s capacity for governing itself and opportunities of
practitioners to develop own identity as well as autonomy, which leads to greater self-
governance in them at an individual level. At this stage, democratic style of leadership proves
effective for developing self-governance in each member, through which they can develop own
specific identities. The second mode of key aspect i.e. co-governance, increases shared
responsibility and develop interaction between team-members, with main goal is to develop an
adequate service provision at community or health-care centre. In this regard, participative style
of leaderships helps in developing specialization and differentiation of services, as well as the

interaction among team-members. This style of leadership includes the criteria of ‘Learning by
doing’ that enables the members for creating a common conceptualization of activity for
promoting well-being and health of people. Other than this, the third key aspect which is
hierarchical governance, is generally characterized through the top-down approach and targeted
activity. To set the goals, design strategies and various activities, as it is essential for health-care
sector, to bring coordination and collaboration among workers in a professional way.
Hierarchical type of governance usually comes when relational change, power coalitions and
more in line with a structural level. For this purpose, hierarchical leadership style will help me in
implementing a functional structure. This will also aid me in establishing a clear chain of
command, for directing each member of multidisciplinary team-members to support each other
and develop own plans for providing best services to team-members. Here, it is essential where I
need to concern more is that each plan must align with overall strategic goal i.e. to promote
health.
P6 Responsibilities of information sharing between multidisciplinary team-working
In delivering personal care at home care and community centre for supporting care to old-
age people, it is responsibility of management of NHS to share essential information among
multidisciplinary team-workers. As working together at such centre is necessary to ensure care
and support therefore, it is essential for management also to carefully defined the different roles
and responsibilities of each team-member who is involved in care. The members included in
home care sectors are staff, care takers, medical practitioners like district nurses; social workers;
and more, where each people works has specific roles and responsibilities which forms a
complex clinical network. Thus, it is essential to implement effective information sharing system
at workplace which will help in providing timely information to each member, to provide
effective timely services and care to patients. This would help in increasing accessibility of
supporting services to patients at home care centers, so that patients can ensure that they are
looked after by an effective team that rise a great sense of confidence in them also. Through
information sharing system, team-members can share views with each other to get second
opinion for providing best care to patients (Fors, 2015). It will prove easier and more beneficial
in providing the continuity of good quality care as well as clinical responsibility for old-age and
disabled people at center with multidisciplinary team. Hereby, constant dynamic towards
effective clinical team will also allow in embedding the clinical management protocols to

provide accessibility of care services. The most advantageous factor of implementing
information sharing practice will be communication among team members where, they can make
timely discussion about clinical matters and obtain solution for providing best treatment for
survival of patients and their well-being. Along with this, team-members also make discussion of
care to patients to get support for recovery and improving their health, which may not be
apparent as possible in one to one clinical management. Thus, best part of information sharing
system is to develop sharing of knowledge that helps in increasing the sense of partnership as
well as develop friendly relationship between team-members to provide effective services and
resolving matters related with management of clinical errors and complaints.

P7 Different communication methods used to provide support to different individuals to meet

Communication refers to sharing of information among associated people in organisation,
who helps in earning high commercial benefit. In other words, it also shows how a company
shares information with each other in order to cater the needs of customers. To meet and review
care needs of people (old age, disabled and other home care patients), it is essential for NHS to
implement appropriate communication system at care centres. For reviewing care needs,
communicating with patients is also considered as important physician competency whereby,
developing interaction with them will help care takers to understand their actual needs. For this
purpose, there are various methods available that management of NHS can be approached such
as –
Interpersonal communication: It refers to a fundamental element for healthcare
interaction, which encompasses with both verbal and non-verbal communication skills.
Here, rapport building and questioning are considered as vital interpersonal skills like
listening, feedback and questioning possess the ability at health-care centres to ensure a
positive relationship between a healthcare professional and patients, for reviewing their
actual care needs. This would aid to reduce barriers between communications and
increase abilities for developing effective relationship among health care practitioners
and patients (Harris and White, 2018).

Non-verbal communication: It is another main effective communication technique,
which is dominated with emotions, for developing communication between service
providers and people who requires special needs of support. It also defines as conveying
one's feeling including facial expressions, body postures and even physical contact
between people. This would help in developing more effective practices between health
practitioners and patients for sharing information related to specific care needs.
Verbal communications: It is the type of effective communication which is always
based on the words in order to share personal information with the other people in an
appropriate manner. But words can be written or spoken for communication in the two
way process. For the better understanding of this communication care homes have been
targeted disabled persons, old age people in order to give them effective support so that
they can give good reviews and homes cares also fulfil their needs and requirement in a
proper way.
Written communication: This communication is involved in the several kind of
messages and information which is prepared by the written words. As it is the most
important and essential part and effective tool of business that always helps companies in
order to take proper decision in an appropriate manner. Therefore, it is important to take
take feedback from every single customer and individual in order to meet consumer's
demand and needs to give them better support while they are taking facilities within the
homes care. That's why it is necessary for making effecting decision within the
marketplace.
As a manager, I used to focus on maintenance of privacy regarding patients personal
data and must ensure that the same will not be disclosed in front of third party, without their
permission. For this purpose, I make sure that hospital must signed written paper with patients
for their allowance and ensure them that their privacy should be maintained. Along with this, by
using both written and verbal communication method, I provide timely information to nurses and
staff members to provide proper care to patients as well as must concern on cleanliness, so that
chance of any discrepancy can be reduced. This will prevent visitors and patients from diseases
that may spread due to uncleanliness and also help in recovery of patients. Information should be
passed to right person and it should not be misused. I provide proper documentation to patients
so that I have all the records safe with me and this information is stored at right place. I use

simple language while taking to patients and hospital staff in order to make my words clear to
them. In this regard, the individuals whom I used to share information for providing best
treatment services in regular manner, by using different communication methods -

P8 Appropriate communication strategies in identifying and responding to the needs of different
service-users in a health, care or support service
In order to identify and respond towards different-service users in health, care and
support service. The best appropriate approach of communication is inter-personal method,
which would not only help in developing strong relationship between service providers and
patients, but also aid to meet their requirement as well (Houghton, 2016). In order to make good
social and health care company has been selected interpersonal communication skills. Wherein,
communication strategies are the part of plan which is mainly used to achieve effective goals in a
proper way. With the help of this employee's of this organisation can easily interact with each
other so that every people can get better help. That's why communication strategies always
identifies the operations and functions of health care and social services which helps them to
make better decision in a good way. In this regard, to analyse needs of service-users in health-
care centres I will behave as active listener i.e. make proper communication with them and
identify what they actually want for living a healthy life. Along with this, to understand the need
I also used to allow patients to take proper time to make decisions about whether they are agreed
to get specified treatment or not, in order to get rid from diagnosed issues. Hereby, I use simple
way of communication style like make interaction with service-users in their own language
basically in English (common language), to gain their trust by cooperating with patients, so that
they can disclose about type of issues. This would help in providing appropriate treatment
services for their well-being. Furthermore, I also use record keeping technique about visitors'
record, type of diagnosed issues, what kind of discussion has made, number of appointments
with same client, to ensure that type of services that have acquired.
Therefore, now this can be said that all of the above discussion about the communication
channels all are effective and attractive for the every social and health care society within the
world. It is necessary because when companies who are comes under the national and social
health care than they should take feedback from their patient in order to know their
improvements and changes in the organisation. This strategy makes them very effective in a
systematic way. By using all these things health care is always available to give support their
patient in order to achieve their demand and accomplish their goals and objectives in a proper
manner. Further more, social and health care organisation need to develop more effective
strategies in order to provide proper services to their customer's in order to effective operations

for senior citizens so that they can feel happy and always shows their potential in the firm in a
proper way (Wildevuur and Simonse, 2015). So that it can be said that all communication
channels are much effective or necessary for the social homes care in order to analyse
themselves as well as able to know the patient query or problems while they are using facilities
which is given by them.
CONCLUSION
From the in-depth analysis of the report, it has been concluded that integrated care
working is effective in providing appropriate health care facilities to the people, thus integrated
care is a collaborative health care service provider for the purpose of providing well-being and
effective health care facilities to the people. It is quite essential for health care organisations to
maintain proper flow of resources like financial aspects, human resources, technological
equipment in respect to providing proper health care benefits to the individuals who are very
much needy. It is also important for the integrated care organisation to implement operations in
the form of an effective structure as to systematic optimisations of resources and facilities. In the
integrated care working, proper communication channels are required to provide suitable
information to each individuals or professionals in respect to providing collaboratively facilities
to the people. In the health care, proper leadership skills are required to be have among the
professionals in respect to conducting proper health care operations by suitable assistance. Thus
this create value for the health care organisations to providing well-being facilities to the people.
