INTRODUCTION
Integration of care at the micro level is concerned with the co-ordination of care for
individual patients and careers. This report highlights local resources which are available to
support the integrated care. After that the report highlights current need related to the health care

Multidisciplinary care used to take place at the time when professionals from a range of
disciplines work together to deliver comprehensive care which can help in addressing as many of
the patient's health and other needs as possible (Fleischman and et.al., 2016). There are many
resources which are available in the market to improve the level of the integrated health care in
the nation some of them are as follow:
Integrated voluntary approach in STP/ICS programme: It is one of the best resources
which is available to improve integrated care in the country. This resource will help the patient in
getting the integrated care at minimum cost as the funding of all this project will be done by
primary and community services to help build ‘primary care networks. These GP-led
partnerships mean people can see an expanded professional team in the local community, which
could include therapists, nurses and mental health staff plus non-clinical support for concerns
like social isolation or debt. This resource will also help the hospital industry and patient in

organization. This will eventually tempt and force them to have a better day to day physical and
mental health and reduce health inequality between different group in the country. This resource

different type of care services HSC need to use different assistive devices that can help them to
provide better health and medical care to patient (Barrington, 2019). Other thing Health care
services need to do, it to use the better methods of promotions to develop understanding in
people who require these service in order to recover from their medical condition. The basis need
HSC have is regarding to the human resources. There numbers of health care officials is not
efficient to provide better care to patients. By this health and social care services can
effectiveness of their services. Availability of skilled and trained staff can improve the process of
personal care and in case of emergency related to health care. This how health care services have
some need in current time in UK.
There are also some changes they need to implement in their current practices. This
changes are basically of technical and procedural type. Technical and procedural changes can
enable to the health and social care services to provide medical care to people with respective
