1.1 Create a testing procedure which explains the 3 tests
you plan to conduct for EACH CLIENT and why are using
these. This should explain how you plan to run each fitness test.
Think about making testing specific to the client’s needs.
Insert/attach your testing procedure documentation
Client 1: For Gary, given his primary condition was his weakened left shoulder, apart
from the primary exercises, the fitness test that was decided to be used was the Back
Scratch flexibility test. This test is used to determine the flexibility and functional fitness
of an individual by measuring the general shoulder range of motion. The equipment
required is a ruler or a yardstick.
This test will be performed prior to the exercises. The participant will be asked to stand
placing right hand behind the head and back over the shoulder, reaching down as far as
possible while touching the back and fingers pointing downwards. The left arm will be
placed behinds the back palm facing outward and fingers pointing upwards attempting to
reach up and touch the tip of the middle finger of the other hand. The distance will be
measured with a ruler in cm. A gap will be indicated by a negative measure and overlap
will be a positive measure.
The test will be repeated by reversing the arm alignments and measuring again.
The rationale for performing this test is to gather information about the current range of
flexibility and movement for Gary’s left shoulder. The reason why the test will be
performed again with changed arm alignment is to understand how the flexibility
measures differ according to different arm alignments
(Results – Left arm above: - 4.5 cm; left arm below: -2.5 cm.)
Client 2: For Daisy, given that her primary objective was to gain mass and strength, the 1
Rep max bench press test was used to evaluate her current strength status.
This test will be performed after the preliminary warm up test. Afterwards the participant
will be explained the technique behind bench press and given a trial lift with a weight-
free bar. Afterwards, a weight will be added and the participant will be asked to attempt
the lift with one repetition. Lowering (2.5 – 5 %) or increasing (5-10%) the lift will depend
upon successful lifting of a weight. After every lift or attempt, the participant will be
made to rest for 2-3 minutes before progressing to the next lift.
Five subsequent lifts after warm up will be counted and the maximum weight lifted (per
body weight) successfully in one rep will be measured for consideration.
Results – Daisy was able to lift a maximum of 8.5 kilograms successfully.

Client 3: For Jeremiah, the primary objective was to reduce his weight as he was
overweight. For that a simple Body Mass Index (BMI) test was done to assess his
condition. The test will be done done prior to performing the exercises.
BMI is calculated from body mass (M) and height (H). BMI = M / (H x H), where M = body
mass in kilograms and H = height in meters. A higher score indicates a higher level of
body fat.
Results – 32.7 (Obese)
TASK 2 - CLIENT SESSIONS
PART A: PRE-EXERCISE SCREENING PROCESS
2.1 Perform the client induction and screening session you have
prepared with each of your clients. Ensure that you allow clients
suitable opportunity and time to ask questions throughout all this
process.
Insert all completed documentation for each client, which
includes:
a. Consent Forms
b. APSS Pre-exercise questionnaires and additional
information questionnaire
c. Nutritional questionnaires
d. Fitness test results
THESE NEED TO BE SIGNED BY THE CLIENT, DATED AND INCLUDED HERE.
2.2 Using the sample Referral Letter write a letter to an allied
health professional regarding one of your client who requires
clearance.

THIS NEEDS MUCH MORE DETAIL
Insert or attach referral letter
Referral letter.
Dear Dr. Mason
This referral letter is in regards to one of my clients, Mr. Jeremiah Louiseau, 59 years of
age, who is suffering from lower back pain and needs medical assistance. As his fitness
trainer and instructor, I have assessed and monitored him for a while and reached the
conclusion that he needs urgent medical assistance from an experienced professional.
Mr Louiseau reports to be suffering from acute lower back pain for the past five – six
months. His pain is most prominent during morning and evening and after long instances
of inactivity. He experiences pain when specific parts of his lower back is pressed. His
previous consultation with his physician has attributed the cause to facet joint
osteoarthritis.
Mr. Louiseau is having trouble performing a lot of his daily activities. He is a heavy
smoker and drinker, however through my sessions he has agreed to cut down on his
habits which will help him in the future. His details are as follows
Age – 59
Sex – male
Height – 173 cm
Weight 98
BMI – 32.7
I request you to provide him clearance to your clinic for further assessment and
treatment.
Thank you
Client 3 Profile information
Name – Jeremiah Louiseau
Sex – male
Age – 59
Height – 5’7’’
Weight – 98kg
Occupation – supermarket owner

Health Appraisal – lower back pain
Objective – weight loss. Eradication of lower back pain.
Tobacco/alcohol/substance use – heavy drinker and smoker.
Primary consultation with his doctor previously has attributed the
cause of lower back pain to Facet Joint Osteoarthritis. Jeremiah
informs that the lower back pain is most pronounced during the
morning and evenings and especially after sitting or standing
without much movement or a long time. He has been experiencing
it for almost the past six months. Besides, when a specific portion
of his lower back is pressed, he also experiences pain. These are
the symptoms of facet Joint osteoarthritis where the pain is caused
by a breakdown of cartilage between the facet joints in the spine.
PART B: ANALYSIS AND DISCUSSING RESULTS
2.3 Students must produce a report that analyses the results of
each clients APSS, fitness assessments and nutritional
questionnaires, then discuss the results with their clients. This
must include:
a) Analysis and discussion with each client the results of their APSS
and fitness tests, then discuss modifications to goals.
