Introduction
Clinical leadership is an integral part of nursing practice as it improves the quality and
standard of care towards any patient or any community. The nurse, as a leader, reviews and
evaluates the process of treatment at an individual level to eliminate the threat of patient
security, avoid medical error, and expand the superiority of patient care distribution (Stanley
and Stanley 2018). The clinical nurse leader also supervises the duty of appointed clinical
nurse in terms of medication delivery, proper setting of intravenous done by the elected
nurse, educate the patient as well as the nurse to maintain a trustworthy relationship. The
common goal of the position is to enhance patient safety. The leader must have a unique set
of delicate qualities such as honesty, courage, creativity, and capability to handle stress
(Joseph and Huber 2015). The primary purpose of the below paper is to identify the role of
the clinical nurse in healthcare based upon a case study and to provide patient care
identifying the priority need.

Role of a registered nurse in a collaborative team
The role of the registered nurse in healthcare is to provide and ensure patient safety by
delivering effective care to the patient and maintaining peace and trustful relationship with
the patient as well as with the healthcare setting (Flinter et al. 2017). The main component in
a collaborative team is effective communication with the patient and also with the other
members (Altman et al. 2016). They can recognize and evaluate the patient clinical condition,
sensitive feeling, and even social needs that help the team along with the patient to avail

funds and thus generating a patient-centered care plan (Fix et al. 2018). There are many
remunerations of collaborative nursing such as it ensures supervision of patient along with
the nurse appointed in duty, it plans the treatment by collaborative knowledge, skills, and
experiences for a critical patient which in turn increases the knowledge of a junior nurse, it
promotes a compassionate working environment, it also highly increases the patient
consequences and satisfactory rate. It also reduces the chance of missed care within the health
care setting, and finally, it reduces the stress level of nurses (Shalala 2014).
Identification of health priorities by Clinical reasoning cycle based upon the case study
The patient in the case study is an older woman who has a history of type 2 diabetes
mellitus, which leads her to develop renal failure. The patient is admitted to the hospital for
the amputation of the left leg. The cause of can condition can be a high level of diabetes
mellitus along with hypertension. The foot ulcer is formed due to the skin tissue breakage,
which affects the bones of the feet in turn—the initial sign of this state swelling and redness
of the foot along with irritation and odor. The black tissues are formed due to ulcers of the
foot because of the absence of blood flow around the affected area. The patient also
developed the condition of gangrene, which is caused due to the severe foot ulcer, and the
state occurs because of tissue death as a result of infection. The symptoms can be severe pain.
The condition has developed due to poor circulation of blood, uncontrolled level of blood
sugar, damage of nerves, and also wounded feet. The patient is an older person and has a poor
economic condition. The improper management of diabetes has to lead the patient in the
development of infectious foot ulcers and renal failure. The immune system of the older
person is less, which makes the body difficult to fight against any infections. The caring plan
must include proper management of blood sugar levels in the body. The body of the older
person is also weak due to nerve damage, which leads to the condition of reducing the
sensitivity of the foot. The circulation of the body is also gets affected as the vascular disease

develops, which makes the blood supply difficult in ulcerous condition. The three most health
priority that needs to be care for this case are management of diabetes mellitus, observation
of the patient before and after the amputation of the leg, and chronic renal impairment. The
nurse and the team must also help the patient with available government resources as she has
a limited source of income. The patient needs complex care, which engages a large number of
plans and treatments.
The amputation needs special care and treatment, which will provide safe and quality
care for the patient. The treatment plan needs a collaborative action of nurses, practitioners,
and also patients' families. The care must be based upon the patient's health status and
relevant external factors such as socio-economical. The nurse must patient must be well
observed, and vital signs must be recorded during the preoperative period. The information
about physical assessment, discussion about the pain of limbs, and previous medications must
be well accounted for. Dressing the operative area regularly is also essential as the patient is
an older person with diabetes, which might lead to the development of other chronic
infections. The nurse must also educate the patient and the family about the post-operative
care and discharge plan by giving special attention in diet plan and movement. Physical
therapy can also be included in the caring plan. The nurse must provide knowledge about the
diet plan, along with medications for the reduction of blood sugar. Drugs such as
phenylalanine, lispro insulin can be recommended for the patient. The renal problem must
also be managed for this purpose. The nurse must guide the patient with daily consumption of
fluid. The skin and body must be monitored due avoid the risk of further development of
ulcers in the body.
The case needs collaboration of a different team of nurses from the preoperative stage
to the post-operative stage. The care and monitoring of patient condition before the operation
such as assessing the vital signs, providing knowledge about the disease and the plan of
