INTRODUCTION
Disability is an illness, condition or injury that makes it more difficult for different
person to do the things that other person do. A long-term conditions is introduced as an illness
that cannot be processed but can usually be handled with medicines and many other treatments.
There are some long-term conditions such as asthma, arthritis, heart failure, diabetes,
hypertension, angina and so on. All these have high impact on different areas of patients life
(Bailly and et. al., 2015). These may cover uspatient's ability to work, family, relationship,
education needs, accommodation and finances. Disability includes different mental and physical
impairments that can decrease or hamper an individual's ability to execute their day to day
activities. It can impact capacity of person to interact and communicate with each other.
Therefore, both long term conditions and living with disability impact on a person's health. For
this report, chosen health and social care organization is Nuffield Health, it was founded in 1957
by Steve Gray and mainly specialize in providing of different products or services namely
nutritional therapy, Hospitals, medical centres, health clinics, rehabilitation centres,
physiotherapy, fitness and wellbeing gyms, occupational health, employee wellbeing etc.
