Introduction
Hypoxia
Oxygen is one of the fundamental factors for human survival, and
without, human beings cannot live. Therefore, oxygen is a key requirement
for body homeostasis. Respiratory and cardiovascular systems are
responsible for oxygen distribution and coordination of homeostasis in the
body. If the two organs are unable to supply a sufficient amount of oxygen to
the tissues in the body, then there may be an imbalance of homeostasis
leading to Hypoxia (Gossman, et .al 2019). Hypoxia occurs when there is an
insufficient amount of oxygen in the tissue either by low oxygen content in
the tissues or poorly oxygenated blood. The disease has a high mortality rate
with a prevalence of 5.7% in small children and elderly people. Relatively the
article reflects on an experience with a patient with a deteriorating condition
during a placement in a nursing home.
Patient Description
After the end of the year semester, each individual in the nursing class had
to go for a placement within the area of study. The tutor identified a nursing
home within the community where I was placed to care for the aged. We
were in groups of 5 students in each placement. It was during the placement
I encountered Mrs. Faith a 75-year-old lady on a clinical deterioration. The
patient was from aboriginal and Torres strait islander community weighing
69kg and height of 169cm. Her husband died five years ago having
succumbed to diabetes at the age of 85 years. Mrs. Faith had three
daughters, who had married abroad though they occasionally visit. The
patient has been smoking for the last 15 years and since then she was
experiencing respiratory failure. Although she has been visiting local
dispensaries for treatment, her condition continues deteriorating day by day.
The patient was to the nursing home with type 1 respiratory failure. She has

been previously diagnosed with septic shock along with whole lung and
pneumonia and lung whole.
Events that led to clinical deterioration
For the last few weeks, Mrs. Faith was feeling unwell and she was not able to
do her daily duties, as she was completely lethargic. Despite her condition,
the patient was denying to have experienced chest pains and rhinorrhea
without any sick contacts. Mrs. Faith had been smoking for the last fifteen
years. Besides in the last ten years, she had been diagnosed with asthma,
tramadol allergy, and pancreatic cancer. From the previous physical
examination, Mrs. Faith was febrile, with dry cough and dispone.
Patient status at the commencement of the shift
Morning shift
After admission the patient was taken to the emergency department during
the morning shift. Where I took care of her. The following cues were
recorded in the patient record book. She was hypoxic, febrile and agitated.
Although there 66%FiO2, The Saturation of oxygen dropped to 83%, ABG,
PaO2-44 and PCO-27 which are a clear indication that Mrs. Faith was hypoxic
and she required to supplemental oxygen. She increased the use of the
Glasgow Coma Scale 10/15.
Evening Shift
In the evening shift, the handover report indicated that she had a pulse rate
of 77bpm, blood pressure 130/61mmHg and a respiratory rate of 24bpm.
Crackles could be easily be heard in the right lung, while saturation of
oxygen was 95% on air pressure. From the abdominal observation during the
patient condition and lab report, she had no abnormalities in the abdomen,
the stool was somehow yellowish, focal consolidation from a chest x-ray as a
sign of pneumonia. The blood test indicated an increase in platelet count.
