INTRODUCTION
Transition plan is the process and is not a one – off event for the teachers, schools and
children who are involved in it. This helps in knowing that the there is certain procedure and
process which is to be followed and this creates value of education and learning process which is
undergone by the students and children (Dockett and et.al., 2021). The timing of activities is also
very important as it ensures that how much the student will invest their time in participating in
activities at large scale.
The transition plan is therefore a whole time process which is being framed to ensure that
teachers and students are taking concern of all the activities and are taking part in them which is
concerned with points of continuity of learning and respect for diversity which helps in engaging
the students and children at large scale. The main aim of transition plan is that it will help the
students and children in knowing and analyzing aspects of growth and development at large scale
which are being taken for education and learning purpose of children. The transition plan helps
on denoting value and knowledge through proper schedules which are being conducted for the
transition of school plan.
RATIONALE
Transition to school plan is the process to help children and students with an Individualized
Education Program (IEP) to help them decide that what they wish to do in high school and it also
helps them figuring out on how to get there. The purpose of the transition plan is to prepare the
children and teens to be independent young adults. For explaining the basis of transitional to
school plan there are three main points to be focused on which will help in knowing the basis
through which all major aspects of how children are able to decided about their future and other
factors are known (Henderson, 2021). The rationale will include major two points which will be
in consideration as to how transition plan is framed at large scale and will consider the reason to
include activities in Transition to School Plan.
These points are described in detail as –
